个案管理模式在结肠造口围手术期患者自我护理能力的应用探讨

摘　　要：目的探讨个案管理模式在结肠造口围手术期患者自我护理能力的应用效果。方法回顾选择2010年2月～2011年2月在我科接受Miles手术的直肠癌患者19例为对照组,按照常规的护理方法进行自我护理能力的指导;2011年3月～2012年3月接受Miles手术的直肠癌患者24例为观察组,应用个案管理模式,由个案管理师对本组患者进行围手术期全程的护理、指导和监督。对两组结肠造口患者早期的自我护理能力进行比较。结果两组结肠造口患者早期的自我护理能力差异有统计学意义(p<0.05),观察组患者出院前造口自我护理能力明显高于对照组。结论应用个案管理模式可明显提高结肠造口围手术期患者早期的自我护理能力,促进患者尽快适应角色的转变,增强回归社会的信心,从而提高生活质量。Objective To explore the effect of the application of case management in patients with self - care during the perioperative period of colostomy surgery. Methods We choose 19 rectal cancer patients who accepted Abdominal perineal resection surgery （Miles＇ resection） from February 2010 to February 2011 as control group, guid- ing self- care ability according to the routine nursing processure; and 24 rectal cancer patients who accepted Miles＇ resection from March 2011 to March 2012 and Care, guidance, supervision coming from Case manager as Observation group. Measure the difference between the two groups in early self - care ability. Results Self - care ability of sto- ma was significantly higher in observation group patients prior to the control group. The difference between two groups is statistically significant in early self- care ability. （p 〈 0. 05）. Conclusion The application of case management does improve the self - care ability of stoma early, promote the role exchange of patients as quickly as possible, and strengthen their confidence to go back society and hence elevate their life quality.
