Campbell入路与尺骨鹰嘴截骨入路内固定治疗肱骨远端C型骨折的比较

摘　　要：目的比较Campbell(肱三头肌舌形瓣)入路与尺骨鹰嘴截骨入路内固定治疗肱骨远端C型骨折的临床疗效。方法 73例肱骨远端C型骨折患者,分别接受Campbell入路内固定治疗(对照组)36例,尺骨鹰嘴截骨入路内固定治疗(观察组)37例,比较两组愈合时间、术后功能恢复及并发症等情况。结果经术后10～24个月随访,观察组功能优良率83.8%,对照组优良率61.1%,观察组功能优良率高于对照组(P<0.05);观察组骨折愈合时间短于对照组(P<0.01)。结论肱骨远端C型骨折采用尺骨鹰嘴截骨入路内固定治疗具有显露充分,术中可直视关节面解剖复位情况、便于内固定操作,有利于肘关节功能早锻炼等优点,是一种较为理想的手术方式。Objective To compare the clinical curative effect between Campbell＇ s posterior approach of elbow（ triceps tongue- shaped flap approach） and oleeranon osteotomy in the treatment of type C fractures of distal humerus. Methods Seventy-three eases of type C fractures of distal humerus who had received operation were retrospectively analyzed,including 36 cases of Campbell＇ s posterior approach of elbow （control group） and 37 cases of olecranon osteotomy （observation group）. The follow-up duration was from ten to twenty-four months. Fracture union time ,function rehabilitation, the rate of complications were evaluated. Results The elbow function excellent rate of olecranon osteotomy （83.8%） was higher than that of Campbell＇ s posterior approach of elbow（61.1% ） in the post- operative follow-up from 10 months to 24 months（ P 〈 0. 05）. The time of fracture union of the patients in the observation group was shorter than that of the patients in the control group（ P 〈 0.01 ）. Conclusion In the treatment of type C fractures of distal humerus, olecranon osteotomy which has the advantages of full exposure, direct observation of joint surface reposition, easy operation of internal fixation and earlier exercises for the function of elbow joint is an ideal approach.
