急性冠脉综合征患者血清亲环素A表达升高的临床意义

摘　　要：目的探讨急性冠脉综合征(ACS)患者血清亲环素A(CyPA)水平变化的临床意义。方法抽取92例ACS患者(A组)、20例稳定性心绞痛患者(B组)及24例对照组(C组)外周血,采用ELISA法检测血清CyPA、基质金属蛋白酶3(MMP-3)、MMP-9水平,免疫透射比浊法测定血清C反应蛋白(CRP)水平。结果 A组血清CyPA、CRP、MMP-3和MMP-9水平较B组和C组明显升高[(31.2±4.8)ng/ml vs.(26.1±3.3)ng/ml和(25.2±4.0)ng/ml、(18.2±2.7)ng/ml vs.(8.7±1.0)ng/ml和(8.2±1.2)ng/ml、(37.4±3.6)ng/ml vs.(18.0±3.7)ng/ml和(16.6±4.3)ng/ml、(61.2±8.0)ng/ml vs.(36.5±5.0)ng/ml和(32.5±5.9)ng/ml](P<0.05或P<0.01)。ACS患者血清CyPA水平与MMP-3、MMP-9和CRP水平均呈显著正相关(r1=0.478、r2=0.424、r3=0.457,P<0.05)。结论 ACS患者CyPA表达明显升高;高表达的CyPA提示冠脉病变的不稳定性。Objective To investigate the clinical significance of cyclophilin A （CyPA） expression in the patients with acute coronary syndrome （ACS）. Methods Peripheral blood was collected from 92 patients with ACS（group A）, 20 patients with stable angina（group B） and 24 healthy people（group C）. Serum levels of CyPA, matrix metalloproteinase-3 （MMP-3） and MMP-9 were detected by ELISA, and the level of C-reactive protein（CRP） was measured by immune transmission turbid^metric method. Results Serum levels of CyPA, CRP, MMP-3 and MMP-9 in group A were all significantly higher than those in groups of B and C[（31.2±4. 8） ng/ml vs. （26.1± 3. 3） ng/ml and （25. 2±4. 0） ng/ml, （18. 2±2. 7） ng/ml vs. （8. 7± 1.0） ng/ml and （8. 2±1.2） ng/ml, （37. 4±3. 6） ng/rnl vs. （18.0±3. 7） ng/ml and （16. 6±4. 3） ng/ml, （61.2±8. 0） ng/ml vs. （36. 5±5. 0） ng/ml and （32. 5±5.9） ng/rnl]（P〈0. 05 or P〈0. 01）. The expression of CyPA in patients with ACS was positively correlated with the expressions of MMP-3, MMP-9 and CRP （rl = 0. 478, r2 = 0. 424, r3 = 0. 457, P〈0. 05）. Conclusion CyPA expression is increased in the patients with ACS, which may indicate the unstability of coronary lesion.
