摘 要:目的评价3T磁共振频率编码方向对垂体高度测量值的影响,以指导垂体磁共振成像参数的选择。方法招募51例健康志愿者(男35例,女16例,平均年龄29岁),使用3T磁共振扫描仪分别于垂直频率编码方向(上/下)和水平频率编码方向(左/有)上进行垂体矢状位T1w1平扫及增强扫描。根据垂体与斜坡的关系,将志愿者垂体图像分为3组:斜坡上型垂体、部分斜坡上型垂体和蝶窦上型垂体。2位神经放射学医师独立地对所有垂体高度进行测量,测量在T1w1增强矢状面图像上进行。数据使用配对样本的t检验分析。结果对于斜坡上型垂体组及部分斜坡上型垂体组,在垂直频率编码方向和水平频率编码方向扫描所得图像的垂体高度值具有显著差异性(P〈0.05)。对于蝶窦上型垂体组,在垂直频率编码方向和水平频率编码方向扫描所得图像的垂体高度值无显著差异性(P〉O.05)。大多数情况下,水平(左/右)频率编码方向扫描所得垂体高度值小于垂直(上/下)频率编码方向扫描所得垂体高度值。结论使用3T磁共振扫描仪进行垂体高度测量,频率编码方向对垂体高度值的测量影响较大,因而在监测患者垂体高度变化时,需注意使用一致的频率编码方向。Objective To evaluate the effects of different frequency-encoding gradient direction(FEG)on measurement of pituita- ry's height with 3T MR scanner. Methods 51 healthy adults (35 male, 16 female, mean age 29 years) were studied prospectively. All of them had taken pituitary MR examinations with 3T MR scanner. The FEG were set as horizontal (left/right) and vertical (in- ferior/superior) separately. MR images included sagittal Tl-weighted images and Tl-weighted Gd-enhanced images. According to the relationship between pituitary and slope, the pituitary MR images were divided into 3 groups:pituitary entirely on the slope,pitu- itary partly on the slope,and pituitary on the sphenoid sinus. Two neuroradiologists measured the height of pituitary independently from all of the sagittal T^-weighted Gd-enhanced images. Paired-sample t test was used to analyse the data. Results For pituitary entirely on the slope group and pituitary partly on the slope group, the height of the pituitaries had significant difference between horizontal (left/right) and vertical (inferior/superior) FEG images (P〈 0. 05). For pituitary on the sphenoid sinus group, the height of the pituitaries had no significant difference between two types of FEG images (P〉0.05). In most of the situation, when the FEG was set as horizontal (left/right), the value of pituitary's height will be less than that when the FEG was set as vertical (inferior/superior). Conclusion Our results indicated that FEG will affect the measurement of pituitary's height in 3T MR images. We had better use the same FEG when following up the patients' height of pituitary with 3T MR scanners.
