摘 要:目的探讨封闭负压引流(VSD)技术在小腿毁损伤治疗中的应用效果。方法对27例小腿毁损伤进行清创、骨折外固定术后VSD治疗。患肢彻底清创后VSD治疗2~3次,肉芽组织覆盖创面给予植皮及皮瓣修复,压力维持-40~-60 kPa。其中24例游离植皮修复,3例采用健侧腓肠神经营养血管蒂皮瓣修复,供区游离植皮修复。结果创面均一期愈合,供区切口一期愈合。患者均获随访,时间12~24个月。皮瓣色泽正常、无臃肿。骨折均愈合,愈合时间8~12个月。结论 VSD技术治疗小腿毁损伤可促进创面肉芽组织生长,为植皮及皮瓣移位修复提供良好的条件。Objective To study the application effect of vacuum sealing drainage (VSD) technique in mangled calf injury. Methods VSD treatment was performed on 27 cases of calf injury after debridement and fracture fixation. The limbs, after thorough debridement, were treated with VSD for 2- 3 times. Skin graft and skin flap were used on the granulation tissue wound, with pressure maintained at - 40 - - 60 kPa. After operation, 24 cases of free skin graft repaired and in the other 3 cases, the free skin of donor site repaired after taking nutrient vascular pedicle skin flap of the contralateral sural nerve repairment. Results Both the wound and the donor site incision healed in the first period. All Patients were followed up for 12 -24 months. The color of flaps was normal and the flaps were not fat. All fractures healed, the healing time ranged from 8 to 12 months. Conclusions VSD technique on mangled calf injury can promote the growth of granulation tissue and provide good conditions for skin graft and skin flap.
