双时相门冬胰岛素30和预混人胰岛素治疗对老年糖尿病患者血糖波动的影响

摘　　要：目的:观察双时相门冬胰岛素30(BlAsp 30)和预混人胰岛素(BHI 30)治疗对老年糖尿病患者血糖波动的影响。方法:64例老年2型糖尿病(T2DM)患者(8mmol/l<空腹血糖<14mmol/l,糖化血红蛋白>8%)随机分为A、B两组,A组予BlAsp 30(诺和锐30),B组给予预混人胰岛素30R(诺和灵30R)进行治疗。用动态血糖检测仪(CGMS)监测患者血糖水平,计算血糖水平标准差(SDBG)、平均血糖水平(MPG)、平均血糖波动幅度(MAGE),空腹血糖变异系数(FPG-CV)以及日间血糖平均绝对差(MODD)作为反映血糖波动指数。结果:两组在血糖达标后,A组空腹、早晚餐后、睡前血糖及HbA1c下降幅度均好于B组(P<0.01或P<0.05),反映血糖波动的指标SDBG、MPG、MAGE、FPG-CV、MODD均低于B组(P<0.05);同时A组夜间低血糖发生率低于B组(P<0.01)。结论:与BH I 30相比,BlAsp 30治疗老年2型糖尿病更有利于血糖稳定。Objective To investigate the blood glucose fluctuation of premixed insulin aspart30 （BIAsp30） with premixed human insulin 30R （BHI30） used in twice-daily injection regimen in patients with eider type 2 diabetes mellitus. Methods Sixty-four elder patients with type 2 diabetes （8mmol/l〈FPG〈14mmol/1, HbAlc〉8%）were divided into two groups ： group A and group B using twice-daily insulin for 12 weeks comparing of BIAsp30 and BH130.The glycemic standard deviation（ SDBG ）, mean blood glucose（MPG） ,mean amplitude of glycemic excursions（MAGE）, fasting plasma glucose-coefficient of variation（ FPG-CV ）,and absolute means of daily differences （MODD） were calculated in each subject. Results After treatment,fasting plasma glucose, palsma glucose after breakfast and after dinner ,bedtime plasma glucose and HbAlc level were significantly lower in group A than in group B（P〈0.01 ~ P〈O.05 ）.The index of blood glucose fluctuation including SDBG, MPG, MAGE, FPG-CV and MODD,meanwhile,hypoglycemia incidence was lower in group A than in group B （P〈O.01）.Conclusion Premixed insulin aspart30（BIAsp30） is better in glycernia stability than premixed human insulin 30R to eider diabetes mellitus.
