摘  要:目的:调查中国人群人类血小板抗原HPA-21w多态性。方法:采用聚合酶链反应-序列特异性引物法(PCR-SSP)基因分型技术,对广州汉族血小板献血者进行HPA-21w基因分型,并使用PCR扩增HPA-21w基因片段,进行DNA测序验证。结果:在200例受检者中发现2例HPA-21a/b杂合子个体,DNA测序表明GPIIIa编码基因1960位G→A,导致GPIIIa膜糖蛋白第628位谷氨酸被赖氨酸所取代,产生HPA-21bw抗原特异性。中国人群HPA-21bw的等位基因频率为0.50%。结论:在中国人群首次检出HPA-21bw等位基因,提示在血小板同种免疫症的诊断和血小板输注治疗中,该抗原具有免疫学意义。Objective:To study the polymorphism of human platelet antigen HPA-21w in the Chinese population. Methods: A total of 200 Han voluntary platelet donors in Guangzhou were genotyped for HPA-21w by using a polymerase chain reaction-sequence specific primers (PCR-SSP) method. In order to confirm the genotyping result, a fragment of HPA-21w gene was amplified and then sequenced directly. Results: Two individuals with a rare HPA-21a/b genotype were found by using PCR-SSP method. DNA-sequen- cing showed that a single G to A substitution at nucleotide 1960 occurred, resulting in the replacement of a glutamic acid with a lysine acid at position 628 of GPIIIa protein. This substitution generated antigenic specificity HPA-21 bw. The allele frequency of HPA-21 bw was 0.50% in the Chinese population. Conclusion: This is the first study reported the rare HPA-21bw allele in the Chinese popula- tion. The detection of two rare HPA-21 bw alleles suggests that HPA-21 bw should he considered in platelet alloimmunization syndromes and platelet transfusion therapy.
