摘　　要：目的研究风湿性心脏病合并慢性心房颤动心房肌中FHL2蛋白水平的变化,探讨FHL2改变在慢性心房颤动中的作用。方法按性别、年龄、二尖瓣狭窄程度、左房大小及肺动脉压力配对,收集接受瓣膜置换手术的风湿性二尖瓣狭窄患者共28例(窦性组14例,房颤组14例)。在建立体外循环时采取右心耳组织及部分慢性心房颤动患者左心耳组织(7例),用Western blot法检测FHL2蛋白在心房肌中的表达水平。结果在右心耳水平,房颤组FHL2蛋白(0.68±0.28 vs 0.51±0.18,t=2.488,P=0.027)表达水平较窦性组明显上调。房颤组FHL2蛋白的表达量在右心耳与左心耳差异无统计学意义(0.58±0.22 vs 0.50±0.13,t=0.984,P=0.363)。结论 FHL2蛋白在风湿性心脏病合并慢性心房颤动心房肌中表达上调,其可能参与慢性心房颤动的发生及维持。而FHL2蛋白在慢性心房颤动左、右心房肌的表达无明显差异,表明右心耳标本替代左心耳在本研究中的结果是可信的。Objective To investigate the expression of FHL2 in chronic atrial fibrillation（CAF） patients.Methods Clinical data were collected from the rheumatic mitral valve stenosis patients received mitral valve replacement.Twenty-eight fresh right atrial appendages（RAA） specimens were collected,among which 14 from patients with sinus rhythm（SR-RAA group） and 14 with chronic atrial fibrillation（CAF-RAA group）.We also obtained 7 left atrial appendages（LAA） in CAF group（CAF-LAA group）.The expression of FHL2 in the RAA and LAA was assessed by Western blot.Results The FHL2 expression in CAF-RAA group was significantly higher than that in SR-RAA group,while there was no significant difference between CAF-RAA and CAF-LAA groups.Conclusion The FHL2 protein is up-regulated in RAA of CAF patients with rheumatic heart disease,suggesting its involvement in the occurrence and maintain of CAF.Our research also shows that the RAA is available for CAF study.
