摘 要:目的探讨晚期糖基化终产物(AGEs)与心肾综合征(CRS)发病的相关性及其与近期预后的关系,进一步探究其在CRS病理生理机制中的作用,为指导临床治疗提供依据。方法以我院186例入院患者及门诊健康体检者为研究对象,根据有无心肾功能损害将其分为空白对照组(A组,51例)、单纯心力衰竭组(B组,48例)、单纯肾衰竭组(C组,46例)和CRS组(D组,41例)。测量4组患者血液中AGEs〔即羧甲基赖氨酸(CML)〕、B型尿钠肽(BNP)、血肌酐(Scr)、胱抑素C(Cys-C)的水平。并通过对D组随访,分析CRS患者近期预后的影响因素。结果 (1)同A、B、C组相比,D组AGEs水平均升高,D组AGEs水平与其余3组比较分别为:(7.38±0.70)mg/L、(0.90±0.34)mg/L、(2.61±1.00)mg/L、(4.37±0.52)mg/L(均P<0.05)。(2)AGEs水平与BNP、Scr、Cys-C均呈正相关,相关系数(r值)分别为0.769、0.723与0.848。(3)AGEs为CRS患者近期预后不良的因素之一(P<0.05)。结论 AGEs与CRS的发病关系密切,且对其近期预后有重要影响,AGEs可能在CRS的病理生理机制中起到重要作用。Objective To investigate the relationship of advanced glycation end - products (AGEs) to cardiorenal syndrome (CRS) to find its role in CRS physiopathologic mechanism to provide basis for its clinical treatment. Methods A to-tal of 186 patients and healthy subjects were divided, according to presence or absence of heart and kidney damages, into groups A ( control group, n = 51 ), B ( with simple heart failure, n = 48 ), C ( with simple renal failure, n = 46), D ( with CRS, n =41 ). The levels of AGEs, B -type natriuretic peptide (BNP), serum creatinine (Scr), cystatin C (Cys-C) were deter-mined. The influencing factors of short - term prognoses of CRS patients were analyzed by follow - up of group D. Results AGEs level was higher in group D than groups A, B, C [ (7.38 ±0. 70) rag/L, (0. 90 ±0. 34) mg/L, (2. 61 ± 1.00) mg/ L, (4. 37 ±0. 52) mg/L, P 〈0. 05]. AGEs were positively correlated with BNP, Scr, Cys-C, the coefficient correlations were 0. 769, 0. 723, 0. 848, respectively. AGEs were one of the adverse factors of short - term prognoses of CRS patients. Conclusion AGEs, closely related to CRS and having important influences on its short - term prognoses, may play an important role in CRS physiopathologic mechanism.
