摘　　要：目的探讨获得性免疫缺陷综合征(AIDS)合并贫血患者外周血细胞特点,探讨影响贫血分类的相关因素。方法选取2010年1—9月河南省3个地区AIDS高发村服用抗反转录病毒(ARV)药物后出现贫血的患者121例,收集其外周血细胞、CD4+T细胞计数及临床资料信息,分析患者外周血细胞特点及其影响因素。结果 121例患者中,52例(43.0%)为单纯贫血者;53例(43.8%)合并单纯白细胞计数减少;9例(7.4%)合并血小板计数减少;7例(5.8%)合并白细胞计数和血小板计数减少。大细胞性贫血66例(54.6%),正细胞性贫血46例(38.0%),小细胞性贫血9例(7.4%)。不同贫血程度患者细胞形态比较,差异有统计学意义(P<0.05)。轻度组较中度组血红蛋白(Hb)、平均红细胞体积(MCV)、平均红细胞血红蛋白含量(MCH)均升高,差异有统计学意义(P<0.05)。单因素分析结果提示,年龄、性别、CD4+T细胞计数、体质指数、婚姻状况对患者贫血程度和细胞形态的影响差异无统计学意义(P>0.05)。结论 AIDS合并贫血患者外周血在Hb减少的同时常合并多系血细胞减少,3种细胞形态的贫血均可发生。Objective To explore the characteristics of peripheral blood cell （PBC） in AIDS patients combined with anemia and the related influencing factors of anemia classification. Methods PBC, CD4^＋ T cell counts and clinical information of 121 AIDS patients with anemia were collected to analyze the features of PBC and its influencing factors. Results Among the 121 AIDS patients with anemia, 52 cases had simple anemia （43.0%）, 53 combined with leucopenia （43.8%）, 9 with thrombocytopenia （7. 4% ）, 7 with leucopenia and thrombocytopenia （5.8%）. Sixty-six patients had macrocytic anemia （54. 6% ）, 46 had positive cell anemia （38.0%）, 9 had small cell anemia （7.4%）. There was significant difference in cell morphology between patients with different degrees of anemia （ P 〈 0. 05 ）. Hb, mean corpuscular volume （MCV） , mean cor-puscular hemoglobin （MCH） were higher in mild group than in moderate group （P 〈 0.05 ）. Univariate analysis results showed that age, gender, CD4^＋ T cell count, body mass index, marital status did not have different influences on the degree or cell mor-phology （ P 〉 0. 05 ）. Conclusion Decrease in hemohglobin is often associated with multi - lineage cytopenia in AIDS patients accompanied by anemia. The 3 cell porphology anemia may occurs.
