摘  要:目的探讨256层螺旋CT在冠状动脉CTA个性化使用对比剂的可行性。方法 2799例CCTA患者病例以体重指数(BMI)分为(BMI<20、20≤BMI<24和BMI≥24)3大组,每组中再根据患者实际体重分为体重<55kg、55kg≤体重<70kg、70kg≤体重<90kg、体重≥90kg的4小组。不同的组内采取向对应的对比剂注射流率完成检查,根据原始轴位图像及重组图像测量12组冠状动脉内造影剂的浓度,并测量评估图像质量。结果冠状动脉在所有病例中均显示良好,12组冠状动脉内的平均CT值、SD,SNR及CNR差异均无统计学意义,经诊断医生的主观评价无统计学意义。结论 256层螺旋CT冠状动脉CTA使用个性化低流率注射对比剂可以满足诊断及临床需要,并大幅降低了X线对比剂的使用剂量。Objective To investigate the feasibility of individualized use of contrast medium in 256-slice spiral CT coronary angiography. Methods A total of 2799 patients who underwent CT coronary angiography were assigned to three groups according to their body mass index (BMI): BMI 〈 20, 20 ≤ BMI 〈 24 and BMI ≥ 24. Each group was then sub-divided into four subgroups according to their body weight: weight 〈 55 kg, 55 kg ≤ weight 〈 70 kg, 70kg≤ weight 〈90 kg, weight ≥ 90 kg. Appropriate injection rate of contrast medium was used for CT coronary angiography in each subgroup. According to original axial images and reconstructed images, the concentration of contrast medium used in 12 subgroups was measured and the image quality was assessed. Results The anatomy of the coronary arteries in all cases was well displayed. There were no significant differences in the mean CT value, SD, signal-to-noise ratio and contrast-to-noise ratio among 12 subgroups, which were consistent with the diagnostician's subjective evaluation outcomes. Conclusion The use of individualized low injection rate of contrast medium in 256- slice spiral CT coronary angiography can meet diagnostic and clinical requirements and greatly reduces the dose of X-ray contrast medium.
