桃红四物汤加五苓散加减对胫腓骨骨折围手术期并发症的影响

摘　　要：目的探讨桃红四物汤加五苓散在胫腓骨骨折围手术期的应用。方法将24例胫腓骨骨折患者随机分成两组,实验组为术前、术后行桃红四物汤加五苓散口服以消肿止痛、活血化瘀,对照组予甘露醇及七叶皂甙钠消肿治疗,两组均行手术治疗,观察入院至手术时间及术后1 d、4 d患肢疼痛情况。结果实验组入院至手术时间为(3.42±1.86)d,对照组为(6.76±2.10)d(P<0.05);术后1 d患肢疼痛、压痛、肿胀度无明显统计学意义(P>0.05),术后4 d患肢疼痛、肿胀度有明显统计学意义(P<0.05)。结论桃红四物汤加五苓散在胫腓骨骨折围手术期使用可促进术前患肢肿胀消退,尽早手术;术后患肢肿胀消退,利于切口愈合,且疼痛缓解便于患肢进行功能锻炼,有较高的临床应用价值。Objective To explore taohong siwu decoction plus Poria Five Powder application in perioperative of frac- ture of tibia and fibula. Methods The 24 patients with tibial and fibular fracture were randomly divided into two groups,The experiment group taken Taohong siwu decoction plus poria five powder to relieve pain, promoting blood circulation to removing blood stasis in perioperative. While the control group with sodium aescinate and mannitol. Treatment of the two groups were observed on the time of preoperative preparation and the limb pain of postoperative lday and 4day. Results The experimental group preoperative preparation time was 3.42 ± 1.86 day, control group was 6.76±2.10 day （P 〈 0.05）. The limb pain, tenderness, swelling didn＇t show significant difference in 1 day postoperative. The limb pain, swelling degree had statistical difference in 4 day ostoperative. Conclusion The application of Tao- hong siwu decoction and poria five powder during perioperation in fracture of tibia and fibula can promote the preoper- ative limb swelling subsided,and can earlier operation. Postoperative promote limb swelling subsided, conducive to the healing of incision. And it can alleviate pain and help the patient to do function exercise, so it has higher clinical application value.
