McCoy杠杆型喉镜在急性呼吸衰竭中实施紧急气管插管的临床应用

摘　　要：目的观察McCoy杠杆型喉镜在急性呼吸衰竭中实施紧急气管插管的应用效果。方法 60例急性呼吸衰竭实施紧急气管插管的患者使用的喉镜不同分为两组:Macintosh标准弯形喉镜(A)组(n=30)、McCoy杠杆型喉镜(B)组(n=30)。A组以Macintosh标准弯形喉镜显露声门,行气管插管;B组以McCoy杠杆型喉镜显露声门,行气管插管。分别观察两组喉镜显露程度,记录一次插管成功次数、插管时间、脉搏血氧饱和度(SPO2)恢复时间,对各项指标进行统计分析。结果 McCoy杠杆型喉镜插管喉显露情况优于Macintosh标准弯形喉镜,一次插管成功率高,插管时间、SPO2恢复时间明显缩短(P<0.01)。结论在急性呼吸衰竭中实施紧急气管插管,McCoy杠杆型喉镜可降低气管插管难度,提高一次插管成功率,缩短插管时间,SPO2可迅速恢复,效果优于Macintosh标准弯形喉镜。Objective To observe the application effect of leveraged McCoy laryngoscope in the emergency endotra- cheal intubation in acute respiratory failure. Methods Sixty patients with acute respiratory failure who received emer- gency endotracheal intubation were randomly divided into two groups, namely the Macintosh standard curved laryngo- scope （A） group （n = 30） and the leveraged McCoy laryngoscope （B） group （n = 30）. Group A used the Macintosh stan- dard curved laryngoscope to expose the glottis and conduct emergent intubation; Group B used the leveraged McCoy laryngoscope to expose the glottis and conduct emergent intubation. The exposure extent, number of times of successful one-time intubation, intubation time and pulse oximetry saturation （SPO2） recovery time were observed and analyzed statistically. Results The leveraged McCoy laryngoscope had superior throat exposure situation, higher successful one- time intubation rate and obviouslY shorter intubation time and SPO2 recovery time compared with the Macintosh stan- dard curved laryngoscope （P 〈 0.01）. Conclusion In the implementation of the emergency endotracheal intubation in acute respiratory failure, the leveraged McCoy laryngoscope reduces the difficulty of endotracheal intubation, improves the success rate of one-time intubation, shortens intubation time and enables fast SPO2 recovery, which is better than the Macintosh standard curved laryngoscope.
