摘 要:目的研究外周动脉硬化指标踝臂指数(ABI)、脉压(PP)与冠状动脉病变的相关性。方法入选200例可疑冠心病患者,均行冠脉造影,将其分为冠心病组和对照组。然后将冠心病组按Gensini积分多少分为轻度病变组(≤20分)和重度病变组(>20分);所有对象均进行ABI及血压测定,比较冠心病组与对照组的差异,并分析ABI、PP与冠心病严重程度的相关性。结果冠心病组ABI较对照组低,重度病变组ABI较轻度病变组低。重度病变组ABI异常发生率高。冠心病组PP较对照组高,重度病变组的PP较轻度病变组高,差异有统计学意义(P<0.05)。Logistic回归分析显示冠脉病变严重程度与ABI呈负相关,与PP呈正相关。结论 ABI、PP是冠心病的危险因素,病变越重,动脉弹性越差。且与冠脉病变严重程度相关,ABI越低、PP越高可能冠脉病变越严重。Objective To investigate the correlation of ankle-brachial index(ABI), pulse pressure (PP) and coronary artery lesions. Methods A total of 200 patients with suspected CHD performed coronary angiography, ABI and PP. The patients were divided into coronary heart disease group and control group. According to Gensini score,coronary heart disease group was divided into mild disease group (≤ 20 points) and severe lesions group (〉20 points). Compar- ing these indexes bentween control guoup and coronary heart disease group,and to analyze the correlation between ABI, PP and the severity of coronary heart disease. Results CHD group ABI was lower than that of the control group, the value of AB! in severe lesions group was lower than in the mild disease group. The abnormal ABI occurred mainly in the severe lesions group, the difference was statistically significant (P 〈 0.05). Logistic regression analysis showed that ABI was negatively correlated with coronary lesions, and PP was positively correlated with it. Conclusion ABI, PP are risk factors for coronary heart disease,the more severe lesions,the worse the arterial elasticity. The lower the ABI or the higher the PP,the more serious coronary artery disease.
