摘  要:目的探讨高频喷射通气在支撑喉镜声带息肉摘除术中的应用价值。方法将60例声带息肉患者按麻醉方法不同分为R组与H组,每组30例。R组采用常规全身麻醉鼻导管吸氧,H组采用全身麻醉高频喷射通气。比较两组通气前后的PaO2、PaCO2、SpO2,同时以手术时间和苏醒时间为评价指标。结果两组通气后的PaO2、PaCO2和SpO2比较,差异有统计学意义(P<0.05),H组的通气后PaO2升高(P<0.05),R组通气后PaO2和SpO2降低(P<0.05),PaCO2上升(P<0.05)。H组患者的手术时间及苏醒时间分别为(9.8±4.4)min、(53.2±14.1)min,与对照组比较差异有统计学意义(P<0.05)。结论高频喷射通气用于支撑喉镜声带息肉摘除术,能够改善手术中麻醉通气不足,安全性好,值得临床推广。Objective To discuss the clinical efficacy of self-retaining laryngoscope extirpating vocal polyp under gen- eral anesthesia and high frequency ventilation. Methods Sixty cases of vocal polyp accoding to the different methods of anmesthesia were divided into routine group (R group) and high frequency group (H group) with 30 cases in each. Patients in R group were carried out with routine general anesthesia and nasal catheter oxygen. Patients in H group were carried out with general anesthesia and high frequency ventilation. The PaO2,PaCO2,SpO2 were compared before and after ventilation in two groups. The operative time and the awakening time were the evaluation indexes. Results There were significant differences between two groups about PaO2, PaCO2, SpO2 after ventilation (P 〈 0.05). The PaO2 was raised observably after ventilation in H group (P 〈 0.05). The PaO2 and SpO2 were reduced observably after ventilation in R group (P 〈 0.05) ,the PaCO2 was increased obviously after ventilation in R group (P〈 0.05). The operative time and the awakening time was(9.8±4.4)min, (53.2±14.1)min respectively and there were significant differ- ences compared to routine group (P 〈 0.05). Conclusion The clinical efficacy and safety was better of general anes- thesia and high frequency ventilation applied to the operation of self-retaining laryngoscope extirpating vocal polyp. It can improve anesthetic of hypoventilation during surgery and hypoxia phenomenon and it is worth clinical promotion.
