贺氏针灸三通法治疗腰椎间盘突出症53例临床观察

摘　　要：目的观察贺氏针灸三通法治疗腰椎间盘突出症(LDH)的临床疗效。方法将106例LDH患者随机分为治疗组和对照组各53例。治疗组风寒痹阻型采用微通法配合温通法治疗,气滞血瘀型采用微通法配合强通法治疗;对照组采用牵引加推拿手法治疗,两组均治疗3周。采用视觉评分表(VAS)、Oswestry功能障碍指数(ODI)及JOA慢性腰痛评分分别于治疗前和治疗后1、3周评价两组患者腰痛程度和日常社会生活活动能力。治疗后3周比较两组临床疗效,治疗后6个月比较VAS、JOA评分。结果两组治疗后1、3周与治疗前比较,VAS、JOA评分及ODI指数比较差异均有统计学意义(P<0.05或P<0.01);两组治疗后1周VAS、JOA评分及ODI指数组间比较差异均有统计学意义(P<0.05)。治疗组临床疗效总有效率92.5%,对照组88.7%,两组比较差异无统计学意义(P>0.05)。两组治疗后6个月VAS、JOA评分与治疗前比较差异均有统计学意义(P<0.01)。结论贺氏针灸三通法治疗LDH可减轻患者局部疼痛,改善临床症状,起效快,且疗效持续稳定。Objective To observe the therapeutic effect of HE＇s three removing obstruction methods of acupuncturemoxibustion for lumbar intervertebral disc herniation （LDH）. Methods Totally 106 cases of LDH were randomized into the treatment group and control group, with 53 in each. The treatment group was given HE＇s three removing obstruction methods of acupuncture-moxihustion according to different patterns. The control group was given massage and traction. The treatment lasted for 3 weeks. The visual analogue scales （VAS）, Oswestry disability index （ODD and Japanese orthopedic association low back pain score （JOA） were used to evaluate the degree of pain and daily social life activity before and after 1-week and 3-week treatment. The therapeutic effect was compared between groups after 3-week treatment. The VAS and JOA scores were followed up in 6 months after treatment. Results Scores in VAS, JOA and ODI in both groups were significantly improved after 1-week or 3-week treatment （P 〈 0.05 or P 〈 0.01） compared with those before treatment. There were significant differences between groups in VAS, JOA and ODI after 1-week treatment （P〈0.05）. The total effective rate of the treatment group and control group was 92.4% and 88.7 % respectively, with no significant difference between groups （P〈 0.05）. There were significant differences in VAS and JOA in both groups （P〈0.01） between before treatment and during follow-up. Conclusion HE＇s three removing obstruction methods of acupuncture-moxibustion can relieve localized pain and clinical symptoms of LDH patients with a rapid onset and better efficacy as compared with traction and massage.
