慢性乙型肝炎病毒感染者不同免疫状态的中医证候特点

摘　　要：目的研究慢性乙型肝炎病毒(HBV)感染者不同免疫状态的证候特点。方法收集慢性HBV感染的免疫耐受、免疫清除和免疫不全患者各200例,制定临床流行病学调查表,调查各组患者一般情况,检测肝功能、乙肝病毒学指标,筛选主要症状及舌脉情况进行中医证候分析。结果免疫耐受组前5型依次为肝郁脾虚、脾气虚、心肝郁热、肝肾阴虚及肝郁气滞;免疫清除组前5型依次为湿热内蕴、肝郁脾虚、肝郁气滞、心肝郁热及气阴不足;免疫不全组前5型依次为肝肾阴虚、气阴不足、脾气虚、肝血虚及肝郁气滞。结论慢性HBV感染不同免疫状态患者涉及中医证候11个,免疫耐受以肝郁脾虚多见,免疫清除以湿热内蕴多见,免疫不全以肝肾阴虚多见。Objective To research the TCM pattern characteristics of chronic hepatitis B virus （HBV） infected patients in different immune status. Methods Totally 200 HBV infected patients were collected and grouped into the immune tolerance group, immune clearance group and immune dysfunction group. The clinical epidemiology survey was designed. The general condition of patients in each group was investigated. The liver function and HBV indicators were detected. The TCM patterns were analyzed by screening main symptoms, tongue and pulse. Results The top five TCM patterns in the immune tolerance group were liver depression and spleen deficiency, spleen qi deficiency, heart-liver depressed heat, liver-kidney yin deficiency and liver depression and qi stagnation. The top five TCM patterns in the immune clearance group were damp-heat brewing internally, liver depression and spleen deficiency, liver depression and qi stagnation, heart-liver depressed heat and qi-yin deficiency. The top five TCM patterns in the immune dysfunction group were liver-kidney yin deficiency, qi-yin deficiency, spleen qi deficiency, liver blood deficiency and liver depression and qi stagnation. Conclusion The chronic HBV infected patients in different immune status are involved with 11 TCM patterns, mainly liver depression and spleen deficiency in immune tolerance, damp-heat brewing internally in immune clearance and liver-kidney yin deficiency in immune dysfunction.
