舒筋洗药配合金黄贴治疗踝关节创伤性关节炎临床研究

摘　　要：目的:观察舒筋洗外用颗粒(简称舒筋洗药)配合天柏金黄散(简称金黄贴)治疗踝关节创伤性关节炎的临床疗效。方法:选取踝关节创伤性关节炎患者40例,随机分为2组各20例。对照组选用扶他林口服,治疗组予舒筋洗药外洗配合金黄贴外贴治疗,2组均以10天为1疗程,治疗2疗程。于治疗前、2疗程后进行视觉模拟评分法(VAS)评分、Mazur评分评价患者疼痛程度和踝关节功能,并对关节液中一氧化氮(NO)含量进行测定。结果:2疗程后,2组VAS评分、NO含量与治疗前相比均降低,Mazur评分升高,经统计学分析,差异均有非常显著性意义(P<0.01)。2疗程后治疗组与对照组相比,在VAS评分和Mazur评分方面,经统计学分析,差异均有显著性意义(P<0.05),在NO含量方面,统计学差异尤其显著(P<0.001)。结论:舒筋洗药配合金黄贴治疗踝关节创伤性关节炎,能显著减轻患者局部疼痛、改善症状,起效迅速、方便安全,可能与其抑制NO在关节液中的表达相关。Objective： To observe the clinical effect of Shujin lotion combined with Jinhuang plaster for the treatment of traumatic ankle arthritis. Methods： Forty patients of traumatic ankle arthritis were equally randomized into control group and treatment group. The control group took Votalin orally, and the treatment group was given fuming and steaming with Shujin lotion plus Jinhuang plaster. Ten days constituted one course of treatment. The visual analog scale （VAS） pain score, Mazur ankle function score, and the content of NO in the synovial fluid were measured before treatment and after treatment for 2 courses. Results： After 2 courses of treatment, VAS score and NO content were lower, and Mazur score was higher in both groups than those before treatment（P〈0.01）. The differences of VAS score, Mazur score and NO content between the two groups after treatment were significant（P〈 0.05 or P〈 0.001）. Conclusion： Shujin lotion combined with Jinhuang plaster is effective and safe for the treatment of traumatic ankle arthritis significantly by relieving pain and other symptoms, and the mechanism may be related to the inhibition of NO expression in synovial fluid.
