摘 要:目的研究中药化痰法能否提高青霉素在慢性阻塞性肺疾病急性加重期(AECOPD)大鼠模型肺组织中的转运能力。方法用熏吸香烟加气管内注射内毒素及鼻腔滴入金黄色葡萄球菌的方法建立大鼠AECOPD模型,80只Wistar雄性大鼠随机分为模型对照组、氨溴索组、清热化痰方组、肺康冲剂组,每组20只。用高效液相色谱-质谱法检测各组大鼠不同时段生物标本中青霉素的浓度。结果 15 min血及肺组织中青霉素浓度比较:分别与模型对照组、肺康冲剂组比较,氨溴索组和清热化痰方组浓度均增高,差异有统计学意义(P<0.01,P<0.05)。30 min血中青霉素浓度比较:与肺康冲剂组比较,清热化痰方组浓度增高,差异有统计学意义(P<0.01)。30 min肺组织中青霉素浓度比较:与模型对照组比较,肺康冲剂组浓度降低,差异有统计学意义(P<0.05),清热化痰方组浓度增高,差异有统计学意义(P<0.05);与肺康冲剂组比较,清热化痰方组浓度增高,差异有统计学意义(P<0.01)。结论氨溴索与清热化痰方均能提高青霉素在AECOPD大鼠模型肺组织中的药物浓度,即能增强其在肺组织中的转运能力,为临床选择中药作为AECOPD抗生素治疗的辅助用药提供了实验依据。Abstract: Objective To study whether or not the dispersing phlegm therapy of Chinese medicinal can improve the transport ability of penicillin in lung tissue in rats with acute exacerbation chronic obstructive pulmonary disease (AECOPD). Methods The rat model of AECOPD was established by using cigarette smoking, intratracheal injection of endotoxin and intranosal instillation of Staphylococcus aureus. Male Wistar rats ( n = 80 ) were randomly divided into control group, ambroxol group, Qingre Huatan Fang group and Feikang Granules group ( each n = 20). The concentration of penicillin in biological samples was detected by using high performance liquid all groups at different time periods. Results chromatography-mass spectrometry (HPLC-MS) in rats of The comparison in the concentration of penicillin in blood and lung tissue after 15 minutes showed that the concentration increased in ambroxol group and Qingre Huatan Fang group compared with control group and Feikang Granules group ( P 〈 O. 01, P 〈 0. 05 ). The comparison in the concentration of penicillin in blood after 30 minutes showed that the concentration increased in Qingre Huatan Fang group compared with Feikang Granules group ( P 〈 0. 01, P 〈 0.05 ). The comparison in the concentration of penicillin in lung tissue after 30 minutes showed that the concentration decreased in Feikang Granules group ( P 〈 0. 05 ) and increased in Qingre Huatan Fang group (P 〈 0. 05 ) compared with control group. The concentration increased in Qingre Huatan Fang group compared with Feikang Granules group ( P 〈 0.01 ). Conclusion Ambroxol and Qingre Huatan Fang (with guiding of the therapy of clearing beat and dispersing phlegm ) all can increase the concentration of penicillin in ling tissue in rat model of AECOPD, which means it can improve the transport ability of penicillin in lung tissue.
