摘  要:[目的]通过观察慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)状态下大鼠肺肾超微结构、肺组织匀浆基质金属蛋白酶-9(Matrix metalloproteinases-9,MMP-9)含量、尿半胱氨酸蛋白酶抑制剂-C mRNA(Cysteine protease inhibitors-C mRNA,Cys-C mRNA)表达水平的变化,探讨COPD状态下肺对肾的影响。[方法]将大鼠随机分为正常组、COPD组、COPD急性加重期组,每组8只,用改良烟熏加气管滴脂多糖法建立COPD模型,气管内滴加脂多糖(Lipopolysaccharide,LPS)造成COPD急性加重模型。酶联免疫吸附测定(Enzyme-linked immuno sor-bent assay,ELISA)法测MMP-9含量,反转录酶-聚合酶链锁反应(Reverse transcription-polymerase chain reaction,RT-PCR)法测尿Cys-C mRNA表达,光镜、透射电镜观察肺、肾病理改变。[结果]与正常组比较,COPD急性加重期组、COPD急性加重期组的MMP-9的蛋白表达及尿Cys-CmRNA表达均显著增高(P<0.01),与COPD组相比,COPD急性加重期组MMP-9及尿Cys-C mRNA表达水平均显著增高(P<0.05),光镜、透射电镜观察,COPD急性加重期组肾损害最严重,其次是COPD组,正常组损害较轻。[结论]在大鼠COPD发病中,肺脏损伤对肾脏有一定的影响,二者呈正相关性,COPD可引起一定的肾损害,提示在COPD病程中肺肾之间的相关性,COPD治疗过程中应注意对肾脏的保护。[Objective] Observe the change of the expression level of rats’ urine Cys-C mRNA,the content of MMP-9 in lung homogenates and the ul trastructure of lung and kidney and discuss the influence of lung on kidney in COPD.[Methods] Divide the rats into three groups randomly: the normal group(N),the COPD group(B)and the acute exacerbation of COPD group(C),with 8 rats in each group.Establish the model of COPD with the improved smoke plus lipopolysaccharide(LPS) dropping in the trachea.Then drop LPS in the trachea again to establish the acute exacerbation model of COPD.Detect the content of MMP-9 using ELISA,the expression of urine Cys-C mRNA with RT-PCR and observe the pathological changes of lung and kidney with light microscope and electron microscope.[Results] Compared with group N,in group C and B,the protein expression of MMP-9 and urine Cys-C mRNA was increased significantly(P&lt;0.01);compared with group B,the protein expression levels of MMP-9 and the expression level of urine Cys-C mRNA in group C was higher significantly(P&lt;0.05).By light microscopy and electron microscopy,the renal damage of group C was most severe,followed by group B,and group N was not apparent.[Conclusion] In COPD disease of rats,damage of lung had certain influence on kidney.There was a positive correlation between them.COPD could do certain damage to kidney,indicating the correlation between lung and kidney in COPD.Therefore,we should pay attention to the protection of kidney when treating COPD.
