范永升教授辨治狼疮性肾炎蛋白尿经验

摘　　要：[目的]总结范永升教授治疗狼疮性肾炎(lupus nephritis,LN)蛋白尿经验。[方法]通过多年侍诊及自身临床实践,从病因病机、治则治法、辨证用药特点等方面出发,整理分析范永升教授治疗LN蛋白尿的特色经验。[结果]范永升教授认为LN蛋白尿是在"毒、瘀、虚"的基础上形成的;治疗应兼顾"辨病与辨证""、整体与局部"";益肾祛瘀解毒"是治疗本病的基本大法;狼疮之毒是在肾虚基础上由外感引发,具有湿温之邪的特点,有类似卫气营血的传变规律;结合临床指标半定量化辨证分型可使用药更有针对性。[结论]运用"益肾祛瘀解毒法"结合半定量化辨证治疗狼疮性肾炎蛋白尿具有较好疗效。[Objective] To summarize professor Fan Yongsheng’s experience in treatment of lupus nephritis proteinuria.[Methods] By learning from profes sor Fan Yongsheng for many years and my clinical experience,from the etiology and pathogenesis,therapeutic principle of TCM syndrome differentiation treatment characteristics,etc,analyse Fan Yongsheng professor’s experience in treatment of lupus nephritis proteinuria.[Results] Professor Yongsheng Fan’s view is that proteinuria resulted from lupus nephritis is formed at the basic＂ toxico stagnation asthenia＂,and should be treated with consideration to two or more things such as differential diagnosis of diseases and differentiation of symptoms and signs,entirely and partly.Tonifying kidney to remove stagnation and detoxication are basic mathods for treating proteinuria resulted from lupus nephritis.Toxico from lupus which is caused by renal deficiency is initiated by six climate pathogenic factors,it has the characteristics of damp thermosis and has similar regular pattern progress of disease such as wei qi yingfen and xuefen;Combining with clinical index of semi quantitative can use drug more targeted.[Conclusion] Using tonifying kidney to remove stagnation and detoxication combined with semi quantitative syndrome differentiation in treating lupus nephritis proteinuria achieves good effect.
