摘  要:目的评估1型糖尿病胰岛素撤退试验对血成纤维细胞生长因子-21(FGF-21)及游离脂肪酸(FFAs)的影响。方法测定1型糖尿病患者胰岛素撤退试验前后血胰岛素、FGF-21、FFAs及其它代谢因子的变化情况,并进行统计学分析。结果试验组血糖、血酮、血FFAs显著增高(P<0.001),血FGF-21显著增高(P=0.02)。对照组血酮显著增高(P=0.04)。试验组胰岛素变化与血FFAs及FGF-21变化无显著相关性。而血FFAs变化与FGF-21变化呈显著正相关(r=0.63,P=0.03)。结论 1型糖尿病FFAs精细调控FGF-21,而且具有相对独立性。胰岛素对1型糖尿病FFAs调控FGF-21不产生实质性影响。Objective Study effect of insulin withdrawal on serum free fatty acids (FFAs) and fibroblast growth factor 21(FGF-21)levels in type 1 diabetes. Methods 30 subjects confirmed diagnosis of type 1 diabetes measured the changes of serum insulin, FFAs, FGF-21 and other metabolic parameters levels before and after the experiments, Then, carry out statistic analysis. Results The serum glucose, ketone and FFAs levels significantly increased (P〈 0.001), serum FGF-21 levels also significantly rose (P=0.02)in experiment group. Furthermore, the serum ketone levels in control group significantly increased too (P=0.04). In experiment group, the changes of serum insulin levels were no statistically relation with that of serum FFAs and FGF-21 levels.However, there were significantly positive correlation between the changes of serum FFAs and FGF-21 levels (r=0.63, P=0.03). Conclusions In type 1 diabetes, FFAs might be responsible for the upregulation of FGF-21, and be considered to have relative independentability. It seems to no substantial impact for insulin to increase the FFA-induced FGF-21 levels.
