盐酸坦洛新缓释片治疗输尿管下段结石的疗效分析

摘　　要：目的评估盐酸坦洛新缓释片治疗输尿管下段结石的排石疗效。方法回顾2010年6月至2011年6月期间的输尿管下段结石病例共126例的临床资料,所有结石均经B超、CT确诊,结石大小4～10 mm,病例随机分成两组:治疗组60例,予口服石韦散中药排石汤每日一剂及盐酸坦洛新缓释片0.2 mg每日一次;对照组66例,单独采用石韦散中药排石汤治疗,两组同时每天饮水约2 L。两组随访期限4周,随访中断为结石排出,记录两组的结石排出时间、结石排出率、肾绞痛发作次数、镇痛药物使用次数及药物不良反应。结果 126例患者均成功随访,治疗组结石完全排出45例,排石率为75.00%;对照组结石完全排出36例,排石率为54.55%;治疗组与对照组在排石率、排石时间、肾绞痛发作及镇痛药物的使用方面均有明显差异(P均<0.05);两组病例均无明显并发症及不良反应。结论盐酸坦洛新缓释片不仅可以有效促进输尿管下段结石的排出和缩短排石时间,并能减少排石过程中肾绞痛的发生及镇痛药的使用。Objective Our study attempted to understand the efficacy of tamsulosin hydroehloride sustained release tablets in treating lower ureteral stones. Method The clinical data of 126 patients admitted to our hospital during January, 2010 and January, 2011 were reviewed. The disease of lower ureteral stones was diagnosed by ultrasound and non-contrast CT. The patients were divided into two groups： an experimental group of 60patients and a control group of 66 patients. The experimental group were given tamsulosin hydrochloride sustained release tablets （0. 2mg per day） and Shiweisan （A traditional Chinese medicine） , while the control group（66patients） were given only Shiweisan. All patients drank water（about 2L per day）. Two groups were followed up for four weeks. Calculus expulsion time, calculus expulsion rate, the times of renal colic episodes, times of using analgesic, and incidence of side effect in both groups were recorded. Findings 126 cases were followed up. The calculus expulsion of 45 cases in the experimental group（75.00% ） was successful, and of 36 cases in the control group（54.55% ）. The experimental group were significantly different from the control group in calculus expulsion time, calculus expulsion rate and the average time of renal colic episodes and using analgesic per person （P 〈 0.05） ; There were no side effect and complications in two groups. Conclusion Tamsulosin hydrochloride sustained release tablets can not only increase the expulsion rate, shorten the expulsion time, but also reduce the incidence of renal colic and rate of using analgesic in treating lower ureteral stones.
