黄连膏配合艾条灸治疗Ⅱ期前压疮的临床疗效观察

摘　　要：目的观察黄连膏外敷配合艾条灸熏烤治疗可疑深部组织损伤、Ⅰ期、Ⅱ期压疮的疗效。方法选取2011年11月-2012年4月入院的可疑深部组织损伤、Ⅰ期、Ⅱ期压疮患者35例(共计42处),随机分为治疗组和对照组,对照组采用每日基础护理支持,治疗组在基础护理之外,每日实施艾灸熏烤10～15 min,行雀啄灸,并每日黄连膏涂抹患处。从压疮大小、创面情况、24 h渗液量、周边皮肤的情况四个方面进行评价和观察。通过对患者初始压疮情况进行评分,治疗10 d后再进行评分,然后对两组两次分数的差值进行比较。结果两组压疮大小、创面情况、周边皮肤的情况比较,差异有统计学意义(P<0.05)。两组24 h渗液量比较,差异无统计学意义(P>0.05)。治疗组总有效率明显高于对照组,差异有统计学意义(P<0.05)。结论采用黄连膏配合艾条灸治疗可疑深部组织损伤、Ⅰ期、Ⅱ期压疮效果明显,值得推广。Objective To observe the efficacy on pressure sores Subspested Deep Tissue Injury,and at stage I and stage II treated with external application of coptis plaster and moxibustion. Methods Thirty - five inpatients of pressure sores Subspested Deep Tissue Injury and at stage I and stage II were collected from November 2011 to April 2012 ,totally 42 sites of pressure sores. They were randomized into a treatment group and a control group. In the control group, the daily basic nursing support was adopted. In the treatment group, besides the basic nursing, moxibustion was applied, 10 to 15min each day, with sparrow- pecking method. Additionally, coptis plaster was used on the affected sites every day. The assessment and observation were carried on in terms of sore size,wounding condition,24 hours amount of exudate and skin condition a- round the affected site. The scores were given at the initial stage of pressure sores and 10 days after treatment separately. The value difference was compared between the two groups. Results The differences in sore size, wounding condition and skin condition around the affected site were significant statistically between the two groups（P 〈 0.05 ）. 24 hours amount of exudate were not different significantly between the two groups （P 〉 0.05）. The total effective rate in the treatment group was higher apparently than that in the control group,in- dicating the statistical significance （P 〈 0.05 ）. Conclusion Coptis plaster and moxibustion achieve the ap- parent effect on pressure sores Subspested Deep Tissue Injury and at stage I and stage II and this therapy de- serves to be promoted.
