摘 要:目的探讨甲状腺肿患者血清细胞因子〔白介素(IL)-2、IL-10〕水平与自身抗体及细胞学类型的相关性。方法选取2010年1月—2011年12月我院体检发现甲状腺肿伴或不伴结节的患者60例。测定各研究对象血清IL-2、IL-10以及甲状腺过氧化物酶抗体(TpoAb)和甲状腺球蛋白抗体(TGAb)水平,并行甲状腺细针穿刺细胞学检查获得病理类型。依次将研究对象分为TpoAb阳性组与TpoAb阴性组、TGAb阳性组与TGAb阴性组、桥本甲状腺炎组与非桥本甲状腺炎组。比较各组血清IL-2、IL-10水平,同时进一步分析TpoAb阳性组IL-2、IL-10与TpoAb的相关性。结果 TpoAb阳性组较TpoAb阴性组IL-2水平升高、IL-10水平降低,差异均有统计学意义(P<0.01)。TGAb阳性组较TGAb阴性组IL-2水平升高,差异有统计学意义(P<0.05);IL-10水平比较,差异无统计学意义(P>0.05)。桥本甲状腺炎组较非桥本甲状腺炎组IL-2水平升高、IL-10水平降低,差异均有统计学意义(P<0.01)。TpoAb阳性组IL-2与TpoAb呈正相关(r=0.977,P<0.01);IL-10与TpoAb呈负相关(r=-0.656,P<0.01)。细针穿刺细胞学诊断桥本甲状腺炎者28例,其中TpoAb阳性26例(占93%),阴性2例(7%);TGAb阳性23例(82%),阴性5例(18%)。非桥本甲状腺炎32例,其中TpoAb阳性12例(38%),阴性20例(62%);TGAb阳性10例(31%),阴性22例(69%)。结论血清IL-2、IL-10水平与TpoAb及甲状腺细胞学类型有相关性,在排除其他影响因素后可作为桥本甲状腺炎与其他病因甲状腺肿的鉴别诊断依据。Objective To investigate relationship among serum IL - 2 , IL-10, autoantibodies and cytology types ingoiter patients. Methods 60 goiter patients with or without nodule from January 2010 to December 2011 were selected. The serum IL - 2, IL-10 , TpoAb and TGAb levels and TpoAb,TGAb were all detected,and meanwhile FNAC was performed. The pa-tients were divided into TpoAb positive group,TpoAb negative group,TGAb positive group,TGAb negative group,HT groupand non - HT group. The IL-2 and IL-10 levels were compared between groups and the relationship between IL - 2, IL-10and TpoAb was analyzed in TpoAb positive group. Results The IL-2 was increased and the IL-10 was decreased in TpoAbpositive group compared with TpoAb negative group, and the difference was statistically significant (P 〈0. 01 ) . The IL-2 wassignificantly increased in TGAb positive group compared with TGAb negative group (P 〈 0. 05 ),but IL-10 showed no suchdifference (P 〉0. 05) . The IL-2 was increased and the IL-10 was decreased in HT group compared with non - HT group,and the difference was statistically significant (P 〈0. 01 ) . In TpoAb positive group,IL-2 was positively correlated with TpoAb(r =0. 977,P 〈0. 01 ) and IL - 10 was negatively correlated with TpoAb (r = -0. 656,P 〈0. 01 ) . FNAC showed that 28 cases had HT, among which 26 cases were TpoAb positive (93% ) , 2 cases were TpoAb negative (1% ) , 23 cases were TGAbpositive (82% ) and 5 cases were TGAb negative ( 18% ) . Among the 32 non - HT patients, 12 cases were TpoAb positive(38% ),20 cases were TpoAb negative ( 62% ),10 cases were TGAb positive ( 31% ) and 22 cases were TGAb negative(69% ) . Conclusion The levels of serum IL - 2 , IL-10, TpoAb and FNAC showed correlation with each other, and the serum IL-2 and IL-10 levels can be regarded as one of the diagnostic indicators to identify HT.
