摘　　要：目的了解安徽省综合性三甲医院的医院感染率水平及其影响因素。方法采用横断面调查方法,以2010年11月10日0～24点当天所有在院病人为研究对象,对医院感染状况进行调查,并分析其影响因素。结果该地区医院感染率为2.47%,例次率为2.63%;下呼吸道、上呼吸道是医院感染常见部位;常见的分离菌分别为铜绿假单胞菌、大肠埃希菌和鲍曼不动杆菌。多因素Logistic回归分析结果显示,医院感染与进行动静脉插管(OR=3.431,95%CI:2.498～4.712)、泌尿道插管(OR=2.089,95%CI:1.522～2.687)、使用呼吸机(OR=1.828,95%CI:1.254～2.667)密切相关。结论安徽省13家三甲医院的医院感染率虽低于全国水平,但仍不容忽视。需继续加强监测,减少不必要的侵入性操作,降低医院感染的危害。Objective To investigate the prevalence rate of nosocomial infection in grade 3 and first-class hospitals in Anhui Province,and to explore the underlying influencing factors.Methods Epidemiological method of cross-sectional study was used.All of the patients in the hospital were selected as participants.Nosocomial infection in the 2010 year of November 10th was under investigation.Results The prevalence rate of nosocomial infection was 2.47%;the case-time infection rate was 2.63% in Anhui area.The lower and upper respiratory tracts were the most common infection positions.The common clinical bacterial isolates were pseudomonas aeruginosa,escherichia coli and acinetobacter baumannii.The results of multiple-factor Logistic regression analysis showed that the nosocomial infection was closely related to arterial and venous intubation（OR = 3.431,95% CI： 2.498-4.712）,urinary catheterization（OR = 2.089,95% CI： 1.522-2.687） and the respirator utilization（OR = 1.828,95% CI： 1.254-2.667）.Conclusions The average prevalence rates of nosocomial infection in all thirteen grade 3 and first-class hospitals was relatively low in Anhui Province.However,the monitoring and controlling work for nosocomial infection is still needed to be strenghen to reduce unnecessary invasive operation.
