小切口皮下剥离加刮除术治疗腋臭的临床疗效观察

摘　　要：目的:评估小切口皮下剥离加刮除术治疗腋臭的临床效果,为腋臭的治疗提供一种新的方法。方法:收集我科2009年10月～2012年5月行小切口皮下剥离加刮除术治疗腋臭患者共98例,男42例,女56例。取平卧位,以腋毛生长区边缘画线确定清除范围,在腋窝行肿胀麻醉。在手术区域中央沿皮纹走向取长1cm切口,分离皮肤和皮下组织。用外科剪剪除汗腺及脂肪组织,再以刮匙刮除残存汗腺及脂肪组织。缝合切缘,弹力绷带以"8"字加压法包扎。结果:98例患者手术均成功。手术时间60～80min。其中1例术后一侧形成血肿,2例拆线后切缘缝合处轻微裂开。经处理后所有98例患者术后均满意。结论:采用小切口皮下剥离加刮除术治疗腋臭具有创伤小、术后处理简单、恢复快、治愈率高的优点,是值得推广的一种手术方法。Objective Evaluating the therapeutic effect of decollement and curettage with a miniincision in the treatment of axillary osmidrosis to provide a new way for the treatment of axillary osmidrosis. Methods Gather 98 cases of axillary osmidrosis treated by decollement and curettage with a mini-incision from October 2009 to May 2012 in our department, 42 cases of male and 56 cases of female. Take horizontal position to determine the armpit hair growing region as the scope of the clear. And prepare swelling anesthesia in this area. Take lcm long incision in the middle of the surgical area along the dermatoglyph, then separate the skin from subcutaneous tissue. Cut off the sweat gland and fat tissue, and then scrape the residual sweat glands and adipose tissue with a curette. Suture the surgical margin and take pressure dressing in ＂8＂ format with elastic bandage. Results Surgery of all 98 cases were successful. The operation time is fall in between 60 and 80min. Among them, 1 case of postoperative hematoma was formatted on one side. In 2 cases, the surgical margin slightly cracked after taking out stitches. By correct administration, the all 98 patients go to postoperative satisfaction. Conclusion Decollement and curettage with a mini-incision has the advantage of small trauma, simple process, high cure rate, and fast recovery after operation. So it is an operation method be worth to popularize.
