摘 要:目的:探讨最小表观弥散系数(ADCmin)值在淋巴结定量分析中的价值。方法:通过回顾性地分析低分化鼻咽癌患者的颈部淋巴结的弥散成像,以最短径1.5~2.5 cm为感兴趣淋巴结,分为未化疗组(31枚)和化疗组(16枚),分别统计两组淋巴结的最短径和坏死率;同时用Image J软件分别测量淋巴结实质部分的ADCmin值和平均ADC(ADCmean)值。结果:两组淋巴结的最小径分别为(2.23±0.24)cm和(2.14±0.44)cm,组间差异无统计学意义(t=0.467,P>0.05);两组淋巴结的囊变坏死率分别为6.45%和43.75%,差异有统计学意义(χ2=7.227,P<0.05);两组的ADCmin值分别为(0.417±0.083)×10-3mm2/s和(0.433±0.758)×10-3mm2/s,组间无统计学差异(t=-0.667,P>0.05);但是ADCmean值分别为(0.745±0.112)×10-3mm2/s和(0.838±0.140)×10-3mm2/s,组间差异有统计学意义(t=-2.467,P<0.05)。结论:在放射性治疗后,ADCmin表达的临床意义可能与ADCmean不同,两者联合可能为临床提供更多有价值信息。Purpose: The aim of the study was to verify the efficacy of minimal apparent diffusion coefficient (ADCmin) in evaluating cervical lymph nodes. Methods: Imaging data of cervical lymph nodes in patients with poorly differentiated nasopharyngeal carcinoma (NPC) were analyzed retrospectively. According to the criteria that the smallest diameter was between 1.5 cm and 2.5 cm, all lymph nodes collected in this study were divided into non-radiotherapeutic group (31 lymph nodes) and radiotherapeutic group (16 lymph nodes). The smallest diameter and rate of centric necrosis were calculated. ADCmin and mean ADC (ADCmean) ofparenchyma in cervical lymph nodes were measured by ImageJ software on ADC image. Results: Between these two groups, the smallest diameters of lymph nodes were 2.23±0.24 cm and 2.14±0.44 cm respectively, which were with no significant difference (t=0.467, P〉0.05); The rates of centric necrosis were 6.45% and 43.75% respectively, which were with significant difference(χ2=7.227, P〈0.05); ADCmin was (0.417±0.083)×10^-3mm2/s and (0.433±0.758)×10^-3mm2/s respectively, which were with no significant difference (t=-0.667, P〉0.05); ADC was (0.745±0.112)×10^-3mm2/ s and (0.838±0.140)×10^-3mm2/s respectively, which were with statistical difference (t=-2.467, P〈0.05). Conclu- sion: ADCmin is different from ADC in assessing changes after radiotherapy, combined ADCmean with ADCmin may give more information for clinical treatment.
