先天性小肠膜式闭锁的影像学诊断

摘　　要：目的:探讨影像学检查对先天性小肠膜式闭锁的诊断价值,提高对本病的认识。方法:回顾性分析17例2011年后收集经手术病理证实的先天性小肠膜式闭锁患儿的影像学资料。4例行腹部X线检查;3例行腹部CT加GI;8例行腹部X线加GI;1例行腹部X线、CT;1例行腹部X线、CT及GI。结果:主要X线检查示单泡征2例,双泡征7例,三泡征2例,多个小液平面3例;闭锁端呈盲袋样8例;肠旋转不良1例;主要CT表现:闭锁处呈截断样5例,门静脉及肠壁积气1例。手术结果示十二指肠降段闭锁7例,水平段起始部闭锁2例,十二指肠与空肠交界处闭锁2例,空肠闭锁5例,回肠闭锁1例;伴肠旋转不良2例。有孔隔膜4例。结论:腹部X线检查对本病诊断有重要价值,CT可作为补充检查手段。Purpose： To study the value of different imaging examinations in diagnosis of congenital intestinal membranous atresia, and to improve the understand of this disease. Methods： Seventeen cases of congenital intestinal membrane atresia, which were confirmed by operation and pathology results, were analyzed retrospectively. Of all these cases, 4 were undergone abdominal X-ray examination; 3 were undergone abdominal CT and GI; 8 were undergone abdominal X-ray and GI; 1 was undergone abdominal X-ray and CT; 1 was undergone abdominal X-ray, CT and GI. Results： The X-ray examination results showed that： 2 cases were with single bubble sign; 7 cases with double bubble sign; 2 cases with three bubble sign; 3 cases with multiple small fluid level; 8 cases with latching end like a blind punch; 1 case with intestinal malrotation. The CT examination results showed that： 5 cases were with truncated atresia, 1 case with pneumatosis of bowel wall and portal vein gas. Operation result showed that： 7 cases were with descending duodenum atresia; 2 cases with atresia at the duodenal atresia horizontal section, 2 cases with atresia at the junction of duodenum and jejunum, 5 cases with jejunal atresia, 1 case with ileal atresia; 2 cases with intestinal malrotation; 4 cases with porous diaphragm. Conclusion： Abdominal X-ray examination plays an important role in diagnosis of congenital intestinal membranous atresia, and CT examination can be used as a supplementary method.
