弹性应变率比值对乳腺良恶性肿块诊断的深入研究

摘　　要：目的:将研究病例按绝经前后进行分组分析,对应变率比值在乳腺肿块中的鉴别诊断价值作进一步研究。方法:分别取与病灶同层正常腺体及病灶旁皮下脂肪为参照组织,对300个乳腺病灶(绝经前组192个,绝经后组108个)进行应变率比值SR1和SR2测定,对照其术后病理结果(恶性120个,良性180个),按绝经前后分组界定其临界值并对其诊断率进行统计学分析。结果:300个病灶分别以绝经前组SR1=2.35和绝经后组SR1=2.52为临界值进行良恶性鉴别诊断,诊断敏感性、特异性和准确性分别为79.2%、90%、85.7%,较未分组前(64.2%、84.4%、77.3%)明显提高,P<0.05。分别以绝经前组SR2=2.6和绝经后组SR2=2.67为临界值对300个乳腺病灶进行良恶性鉴别诊断,其敏感性、特异性和准确性分别为59.2%、77.8%、70.3%,较未分组前(70%,73.9%,72.7%)无明显差异,P>0.05。结论:按绝经前后进行分组诊断,弹性成像参数SR1对乳腺良恶性肿块的诊断价值显著提高。Purpose： To explore the value of strain ratio （SR） in the differential diagnosis of breast lesions. Methods： Three hundred patients with breast lesions （180 malignant and 120 benign ones）, which were confirmed by pathological results, were examined with elastographic ultrasound preoperatively. The strain ratio （SR1 and SR2 were calculated using the normal breast tissue of the same-level with the lesion and the subcutaneous fat tissue as reference） of all the breast lesions were recorded. The cutoff values of benign and malignant lesions were obtained in the premenopausal and postmenopausal groups respectively. And their diagnostic performances were then analyzed. Results： When the modified SR1 cutoff value of 2.35 and 2.52 were used in the groups of premenopausal and postmenopausal respectively, the sensitivity, specificity and accuracy were remarkably improved （79.2%, 90%, 85.7% vs. 64.2%, 84.4%, 77.3%, P〈0.05）. No statistical differences were found between the diagnostic value of SR2 with or without the influence of menopausal factor （sensitivity, specificity and accuracy： 59.2%, 77.8%, 70.3% vs. 70%, 73.9%, 72.7%）. Con- clusion： The diagnosis value of elastic strain ratio can be into two groups by the condition of menopause. remarkably improved after the patients are divided
