摘 要:目的探讨实时三维超声心动图(RT-3DE)左心房容积指数(LAVI)评价2型糖尿病患者左心房功能。方法选取2012年2~11月南方医科大学附属深圳宝安医院就诊的2型糖尿病患者50例为2型糖尿病组,选取同期健康体检者35例为对照组。比较两组左心房最大容积指数(LAVImax)、最小容积指数(LAVImin)、收缩前容积指数(LAVIpre)、被动排空容积指数(LAVIp)、总排空容积指数(LAVIt);RT-3DE测量二尖瓣口舒张早期最大流速(E峰)、二尖瓣环舒张早期运动速度(e),计算E/e值;测量左心室射血分数。结果 2型糖尿病组空腹血糖[(9.26±3.75)mmol/L]、糖化血红蛋白[(12.6±4.37)%]、E/e值(12.53±4.58)、LAVImax[(37.42±8.02)mL/m2]、LAVIpre[(28.24±6.67)mL/m2]、LAVImin[(15.43±5.39)mL/m2]、LAVIt[(21.21±5.57)mL/m2]、LAVIa[(11.07±4.44)mL/m2]高于对照组[(4.52±1.19)mmol/L、(5.20±1.43)%、(12.53±4.58)、(26.92±6.16)mL/m2、(19.15±5.31)mL/m2、(13.75±4.74)mL/m2、(15.65±4.83)mL/m2、(7.64±2.57)mL/m2],LAVIp[(9.17±3.38)mL/m2]低于对照组[(9.73±3.77)mL/m2],差异均有统计学意义(P<0.05或P<0.01)。LAVImax、LAVIpre、LAVImin、LAVIt、LAVIa与E/e呈显著相关性(P<0.05或P<0.01)。结论 RT-3DE的LAVI能定量测量2型糖尿病患者左心房储备功能、泵功能、管道功能,有望成为评价左心房功能的新方法。Objective To assess left atrial function in patients with type 2 diabetes mellitus using real-time three-dimensional echocardiographic (RT-3DE) left atrial volume index (LAVI). Methods A total of 50 patients with type 2 diabetes mellitus and 35 healthy individuals from February to November 2012 in the Affiliated Shenzhen Bao'an Hospital of Southern Medical University were selected and divided into 2 diabetes mellitus group (n = 50) and control group (n = 35). LAVImax, LAVImin, LAVIprc, LAVIp, LAVIt were compared between the two groups; E peak and e value were detected by RT-3DE; Left ventricular ejection fraction were detected too. Results Fasting blood-glucose (FPG), glycosylated hemoglobin (HbA1c), E/e value, LAVImax, LAVIpre, LAVImin, LAVIt, LAVIa in 2 diabetes mellitus group [(9.26±3.75) mmol/L, (12.6±4.37)%, (12.53±4.58), (37.42±8.02) mL/m^2, (28.24±6.67) mL/m^2 (15.43±5.39) mL/m^2, (21.21±5.57) mL/m^2, (11.07±4.44) mL/m^2] were higher than those in control group [(4.52±1.19) mmol/L, (5.20± 1.43)%, (12.53±4.58), (26.92+6.16) mL/m^2, (19.15±5.31) mL/m^2, (13.75±4.74) mL/m^2, (15.65±4.83) mL/m^2, (7.64+2.57) mlJm^2], LAVIp in 2 diabetes mellitus group [(9.17±3.38) mL/m^2] was lower than that in control group [(9.73±3.77) mL/m^2], the differences were statistically significant (P 〈 0.05 or P 〈 0.01). The correlation were found in E/e value and LAVImax, LAVIpre, LAVImin, LAVh, LAVIa (P 〈 0.05 or P 〈 0.01). Conclusion LAVI of RT-3DE can quantitatively measure the reserve function, pump function, pipeline function of left atrium; RT-3DE LAVI is a potential new method to evaluate left atrial function.
