基本型乡镇卫生院辖区居民卫生服务利用现状分析

摘　　要：目的分析基本型乡镇卫生院——大别山区胜利镇卫生院所辖农村居民的健康状况及卫生服务需求与利用情况,为改善山区农村居民健康状况提供参考意见。方法采取典型调查的方式,在大别山区选择胜利镇为调查地点,共抽取山区农村居民3 809人。结果居民两周患病率为31.4%,慢性病患病率为20.5%,均高于全国平均水平;不同年龄、不同文化程度居民的两周就诊率差异具有统计学意义;不同性别、不同婚姻状况、不同文化程度居民的慢性病患病情况差异有统计学意义;居民两周患病未就诊、应住院未住院的首要原因均是经济困难;被调查居民接受过免费医疗服务的比例很低。结论为促进农村地区卫生事业的发展,建议加强特定人群的健康教育、增强乡镇卫生院和村卫生室提供服务的针对性、加强免费医疗服务的开展。Objectives To analyze the health status and utilization of rural residents under the framework of the basic health system, and provide a reference for the construction of rural medical service system oriented to basic medical services feature. Methods By means of typical survey, 3 809 rural residents were taken in the Shengli town for the survey. Results The two-week prevalence rate was 31.4% and the chronic diseases prevalence is 20.5 %, both higher than the national level. There was a significant difference in two-week consultation rate among different ages and different education levels. There was a significant difference in chronic diseases rates among different gender, marital status and cultural levels; residents＂ two-week prevalence treatment, the primary reason of people non hospitalized but should be hospitalized and people not seeing a doctor when sick were economic difficulties; free medical services ratio was too low. Conclusions To improve the health Service utilization of rural residents, we sug gest strengthening the health education for specific groups; enhancing the direction of services at the township health center and the village clinics; increasing the form and frequency of free medical service.
