全直肠系膜切除与双吻合器技术在低位直肠癌保肛术中的应用疗效

摘　　要：为探讨全直肠系膜切除（TME）与双吻合器技术（DST）在低位直肠癌保肛术中的应用价值，我们应用TME与DST对120例低位直肠癌行保肛手术。结果显示，全组无手术吻合失败及手术死亡病例。术后病理检查下切缘癌细胞阴性118例，阳性2例。术后吻合口漏4例，经冲洗吸引、对症处理后2周愈合；切口感染10例，局部创口换药后愈合；吻合口狭窄10例，经1个月扩肛后缓解。术后随访12～60个月，局部盆腔复发2例。结果表明，TME联合DST用于低位直肠癌保肛术安全可靠，疗效肯定。This study was to explore the applied value of total mesorectal excision（TME） combined with double stapling technique（DST） for saving anus in low rectal cancer surgery, 120 cases with low rectal cancer were treated with this therapy. As results, without anastomosis failure and death cases were found in the whole group. Postoperative pathologic examination on cutting edge for cancer cells found that 118 cases were of negative results cases and 2 positive ones. Postoperative anastomotic leakage occurred in 4 cases who got cured two weeks after suction and washing and symptomatic treatment, wound infection 10 cases who got healing after local dressing change,anastomotic stenosis 10 cases who got improved after anal dilation for one month. Postoperative follow up of 12-60 months found that 2 cases with local pelvic cavity relapse. The results show that TME combined with DST therapy is safe and reliable, and of certain effects in saving anus of low rectal cancer.
