留置导尿截瘫患者泌尿系感染相关因素调查分析

摘　　要：目的调查分析截瘫患者留置导尿致泌尿系感染的相关因素,并对预防措施进行讨论。方法对2010年1月—2012年12月,122例留置导尿的截瘫患者泌尿系感染状况进行回顾性调查分折。结果 122例患者在尿管留置期间共有112例患者发生菌尿症,总发生率为91.8%,共检出病原菌118株;截瘫患者留置导尿致泌尿系感染的发生率与性别、年龄以及留置时间有关,男性患者明显多于女性,年龄>60岁者较≤60岁的患者感染率高,随着留置导尿时间的增加感染率逐渐增高。结论为了减少截瘫患者留置导尿致泌尿系感染的发生,除掌握留置导尿适应证外,应严格无菌操作,尤其是对男性、年龄较大的患者,尽可能缩短尿管留置时间;在留置导尿期间,要加强导尿管的护理工作,并给予膀胱冲洗等预防性治疗。Objective The study is to investigate the factors concerning urinary tract infection among paraplegics with indwelling catheter,and discuss preventive measures.Methods A retrospective investigation was made among 122 cases from January 2010 to December 2012.Results 112 cases（91.8%）showed bacteriuria with118 pathogens.The incidence of urinary tract infection correlates with sex,age and the duration of indwelling catheter,with more male cases infected than female cases.The paraplegics aged over 60 had higher incidence than those under 59.And the longer duration of indwelling catheter,the higher incidence.Conclusion To lower the incidence of urinary tract infection caused by indwelling catheter depends on stringent aseptic operation and the knowledge of the indication of indwelling catheter.For the male and older paraplegics,the duration of indwelling catheter must be made as short as possible.In the duration of indwelling catheter,preventive measures must be taken such as caring the urethral catheter and rinsing the urinary ladder.
