集束化干预策略在预防乳腺癌术后腋下引流管感染中的应用

摘　　要：目的探讨集束化干预策略在预防乳腺癌术后腋下引流管感染中的应用,为其临床防治提供可参考依据。方法共纳入52例乳腺癌术后腋下引流管引流患者,采用随机数字法平均分为两组,常规组患者给予常规治疗及护理措施;干预组患者给予集束化干预策略,分析两组患者发生引流管感染的差异。结果集束化干预组患者发生感染1例,感染率为3.8%,常规组患者发生感染7例,感染率为26.9%,两组差异有统计学意义(P<0.05);同时集束化干预组患者拔管时间(1.6±0.5)d、住院时间(9.6±3.2)d明显低于常规组患者拔管时间(3.3±1.5)d、住院时间(13.3±4.5)d,两组差异有统计学意义(P<0.05)。结论采用集束化干预策略可有效预防乳腺癌术后行腋下引流的患者发生引流管的感染,可以缩短患者的拔管时间及住院时间,值得临床推广应用。OBJECTIVE To explore the application of the cluster interventions in the prevention of axillary drainage tube infections after the breast cancer patients so as to guide the clinical treatment.METHODS A total of 52cases of breast cancer patients,who underwent the axillary drainage catheterization,were enrolled in the study and randomly divided into two groups;the group A was given the conventional treatment and nursing measures,while the group B was treated with the cluster interventions,then the incidence of drainage tube-related infections was compared between the two groups.RESULTS The infections occurred in one case in the cluster intervention group with the infection rate of 3.8%and 7cases in the conventional treatment group with the infection rate of 26.9%,the difference between the two groups was statistically significant（P〈0.05）.The extubation time of the cluster intervention group was（1.6±0.5）d,significantly shorter than（3.3±1.5）d of the conventional treatment group,and the length of hospital stay of the cluster intervention group was（9.6±3.2）d,significantly shorter than（13.3±4.5）d of the conventional treatment group,the difference between the two groups was statistically significant（P〈0.05）.CONCLUSION Using cluster intervention strategy can not only effectively prevent axillary drainage tube infections in the patients after breast cancer surgery but also shorten the extubation time as well as the length of hospital stay,which is worthy to be promoted in the hospital.
