肘关节恐怖三联征的手术治疗

摘　　要：目的:探讨外科手术治疗肘关节恐怖三联征的治疗效果。方法:选取骨科收治肘关节恐怖三联征患者50例为研究对象,均行外科手术治疗。根据手术入路方式不同,分为肘外侧入路组24例,肘后入路组15例,肘前内侧入路组11例。统计三种入路手术后骨折的愈合情况、术后并发症发生情况以及肘关节功能恢复情况,以此评价外科手术治疗肘关节恐怖三联征的治疗效果。结果:术后患者骨折全部愈合,发生并发症11例。术后患者前臂旋转平均(108±15.87)°,肘关节屈伸平均(122±13.46)°,依据Mayo评分,优37例,良7例,可5例,差1例。结论:肘关节恐怖三联征治疗以重建肘关节同心圆中心复位以及稳定性为重点进行治疗可提高并保证患者预后,同时注意手术并发症的发生。Objective To investigate the clinical etticacy of surgical treatment of elbow joint terrible triad. Methods 50 cases of elbowjoint terrible triad patinets admitted in our hospital were chosen in this study and received surgical treatment. According to the difference of the surgical approach, the patients were divided into elbow lateral approach group （24 cases）, elbow approach group （15 cases） and elbow anteromedial approach group （ 11 cases）. After counting the three approach surgery ,the clinical efficacy of the surgery for elbow joint terrible triad was evaluated as the healing,postoperative complications and functional recovery of the fracture. Results After the surgery,all the fractures were healed, but 11 cases of postperative complications happened. The froream rotation of the patients after the surgery was with an average of （108 ＋ 15.87） degrees. According to the Mayo score,there were 37 cases in excellent,7 cases in good,5 cases in fair,and 1 case in poor. Conclusion Reset and stability of the elbow concentric center focus treatment for elbow joint terrible triad reconstruction can improve and ensure patient prognosis ,meanwhile,the incidence of surgical complications should be noted.
