凝血酶雾化吸入治疗小量咯血的疗效观察

摘　　要：目的:观察凝血酶雾化吸入对小量咯血的治疗疗效。方法:选取我院呼吸内科84例小量咯血患者,随机分为试验组(A组、B组)和对照组(C组)各28例,对照组给予抗感染、止血(氨甲环酸、安络血)、休息、适当镇静、镇咳治疗,试验组在此基础上A组给予凝血酶500 U加入NaCl溶液4 ml雾化吸入,3次/d;B组给予垂体后叶素治疗,比较三组治疗效果及临床不良反应。结果:试验组治疗疗效明显优于对照组(P<0.05),其中A组与B组疗效比较差异无统计学意义(P>0.05),但A组全身不良反应明显小于B组(P<0.05)。结论:凝血酶雾化吸入治疗小量咯血疗效确切,全身不良反应少,优于垂体后叶素。Objective To observe efficacy of thrombin inhalation to therapy small amount of hemoptysis. Methods In our hospital,84 small amount of hemoptysis patients from respiratory medicine department, and they were randomly divided into 3 groups （28 patients for each group） ,then A,B is experimental group and C is reference group. The reference group C received basic treatment as Anti -infective, Hemostatic （ Tranexamic acid, Adrenobazon）, Rest, Appropriate sedation, Antitussive. Then besides basic treatment, experimental group A add atomizing inhalation of Thrembin 500 unit with saline 4 ml,3 times per day. Experimental group B add Pituitrin treatment. Comparing three groups of treatment effficacy and Clinical side effects. Results Treatment efficacy of Experimental group A and B is much better than reference group C （P 〈 0.05 ） , and efficacy is not significant difference between group A and group B （P 〉 0.05 ） , but for Systemic side effects, group A is much less than group B （ P 〈 0.05 ）. Conclusion Efficacy of thrombin atomizing inhalation to therapy small amount of hemoptysis is significantly positive,with very little systemic side effects,so it＇s a better method than Pituitrin treatment.
