军队特勤人员住院疾病谱调查及预防对策

摘　　要：目的调查军队特勤人员的住院疾病谱构成,为提高护理卫勤保障能力、降低特勤人员相关疾病发病率提供理论依据。方法提取解放军第401医院特勤科2008-2012年军队特勤人员住院患者病案首页信息库,参照国际疾病分类标准ICD-9对研究对象住院疾病进行系统分类并进行顺位排列;分别对海潜人员组与飞行人员组疾病按系统分类及按病种分类前6位进行描述,比较两组人员住院疾病按系统分类构成比的差异。结果住院疾病按系统分类总体排名前6位是颈腰椎疾病、消化系统疾病、外伤性疾病、心血管系统疾病、呼吸系统及神经系统疾病;住院疾病按诊断分类单病种总体排名前6位的是腰椎间盘突出症、慢性胃炎、高血压、上呼吸道感染、膝关节损伤及颈椎病。海潜人员组和飞行人员组住院疾病按系统分类构成比进行比较:颈腰椎病、消化系统疾病、外伤性疾病及心血管系统疾病构成比两组间差异无统计学意义(P>0.05);呼吸系统疾病及皮肤疾病构成比海潜人员组高于飞行人员组,神经系统疾病及泌尿系统疾病飞行人员组高于海潜人员组,差异均有统计学意义(均P<0.05)。结论明确军队特勤人员常见病及多发病构成,并提出针对性预防护理措施,对提高卫勤保障能力及军队特勤人员战斗力有重要意义。Objective To investigate the diseases spectrum of hospitalized military special serviceman, so as to provide theoretical foundation for improving nursing ability of medical support and decreasing the morbidity of military special serviceman. Methods Samples derived from a hospital from January 2008 to December 2012 were classified and sequenced according to ICD-9. The top six single diseases of submariner and flying personnel sequenced according to the system and diseases classification were described, so as to compare the differences of constituent ratio according to the disease system. Results The top six disease systems included cervical and lumbar spine disorder, digestive, injury, circulatory, respiratory and neu- ropathy diseases; and the top six single diseases included protrusion of lumbar intervertebral disc, chronic gastritis, hypertension, upper respiratory tract infection, knee joint injury and cervical spinal disorder. No significant difference was found between submariner and flying personnel for constituent ratio of cervical and lumbar spine disorder, digestive, injury, circulatory diseases （P〉0.05）. The percentage of respiratory and skin diseases was higher, while the percentage of neuropathy and urinary system diseases were lower in submariner than those in flying personnel（P〈0.05）. Conclusion Be clear of the disease spectrum for hospitalized military special serviceman may contribute to formulating the plan of military special service-man medical service and raising military special serviceman fighting capacity.
