摘 要:目的探讨助产士分级授权管理对产房质量控制的影响。方法 2012年起,同济大学附属第一妇婴保健院根据助产士的工作年限及技术水平对其进行能力评价及分级授权管理,并比较分级授权管理前后产房的各项质量控制效果(如会阴侧切、伤口愈合不良、会阴Ⅲ度撕裂)、产妇满意度、助产士职业成就感等。结果实施分级授权管理后,会阴侧切率显著下降、会阴Ⅲ度撕裂发生率减少、产妇满意度提高、助产士职业成就感增加,与未进行分级授权管理前相比,差异均有统计学意义(均P<0.05)。结论对助产士进行分级授权管理,有助于降低产妇会阴侧切率,提高产房护理质量。Objective To explore the effects of midwives hierarchical management mode on improving the quality control of the delivery room. Methods The hierarchical management mode was implemented according to the working time and technique skills of midwives since 2012 ,quality control effect (episiotomy,poor wound heal-ing,three degree perinea laceration),parturient' satisfaction and sense of occupation accomplishment for midwives were analyzed and compared before and after implementation of the hierarchical management model. Results Epi-siotomy rate, three degree perinea laceration were significantly decreased(P〈0.05), the parturient satisfaction and occupation accomplishment of midwives were significantly inereased(P〈0.05)after the implementation of midwives hierarchical management mode. Conclusion The hierarchical management mode was helpful for reducing episiotomy rate and improving the nursing quality.
