中药金芪降糖片治疗糖调节受损的系统评价

摘　　要：目的:评价金芪降糖片治疗糖调节受损(IGR)的临床效果。方法:检索Cochrane图书馆、Pubmed数据库、CBMdisc数据库、CNKI数据库、VIP数据库、万方数据库,纳入金芪降糖片治疗IGR的随机对照试验(RCT),并进行方法学质量评价,采用RevMan 5.0软件进行统计分析。结果:共纳入10个RCT。Meta分析结果显示:金芪降糖片对IGR的有效率明显高于运动饮食法(P<0.000 1),其降低血糖效果比运动饮食法和健康宣教显著(P<0.01),但不及二甲双胍、糖脉宁(P<0.05)。其能显著降低FINS、2 hINS,效果与二甲双胍相当(P=0.61,P=0.85);其降低FINS效果与运动饮食法没有明显差异,但降低2 hINS优势明显(P<0.000 01);应用金芪降糖片幷严格运动饮食控制,效果较运动饮食法更为明显(P<0.01)。金芪降糖片对TC、TG、HDL-C的影响也优于二甲双胍(P<0.05)。长疗程(≥12月)应用金芪降糖片或者与运动饮食法以及健康宣教配合,可以显著提高NGR转化率和IGR维持率,降低DM转化率。金芪降糖片的ADR发生率为1.60%,发生率低,反应程度轻微,均为轻度的恶心、腹泻等。结论:基于当前证据,金芪降糖片治疗IGR疗效较好,不良反应少,是治疗IGR有效的中成药。OBJECTIVE To assess the effectiveness and safety of Jiqi Jiangtang tablet in treatment of impaired glucose resistance（IGR）.METHODS Studies involving randomized controlled trials（RCTs） were collected from the database,such as the Cochrane Library,Pubmed,CBMdisc,CNKI,VIP and Wangfang from the year they founded to June 2012.After the necessary methodological quality evaluation of the selected studies,the statistic analysis was conducted with the software Rev Man 5.0.RESULTS Ten RCTs were included.The Meta-analysis results showed that： Jinqi Jiangtang tablets on IGR had higher efficiency than exercise/diet intervention,which significantly decreased blood glucose level.The efficiency was less than those of metformin and Tangmaining（a traditional Chinese medicine）（P0.05）,but showed a very obvious advantages compared with exercise/diet intervention and health education（P0.01）.Studies have also shown that Jinqi Jiangtang tablets could significantly decrease the FINS and 2hINS,the effect was the same as metformin.Jinqi Jiangtang tablets also decrease TC,TG,HDL-C,which effect was better than that of metformin（P0.05）.Long term adminstration（≥12 months） of Jinqi Jiangtang tablets or exercise/diet intervention and health education,could obviously increase the NGR ratio,IGR maintenance rate,and reduce DM conversion rate.The incidence of ADR of Jinqi Jiangtang tablets was 1.60%,and clinic manifestations of ADR were mild nausea,diarrhea.CONCLUSION Current evidences show that Jinqi Jiangtang tablet,is effective and safe in treating IGR.
