摘　　要：目的观察早期低脂肠内营养(EN)治疗对急性重症胰腺炎(SAP)血清三酰甘油水平的影响。方法将34例SAP并发高脂血症患者随机分为试验组和对照组,给予内镜下置肠管早期肠内营养治疗,16例试验组患者给予低脂要素肠内营养,18例对照组患者给予标准营养液肠内营养治疗,观察治疗前后三酰甘油(TG)、白细胞(WBC)、C反应蛋白(CRP)、天冬氨酸转氨酶(AST)、血钙及血糖的变化。结果两组患者治疗后三酰甘油水平均明显下降(P<0.05),但试验组与对照组比较三酰甘油水平下降更加显著(P<0.05),且WBC、CRP、AST、血钙与对照组比较下降更加明显(P<0.05),治疗后血糖下降显著(P<0.05),但两组比较差异无统计学意义(P>0.05),考虑与均采用胰岛素降糖治疗有关。结论低脂EN能够有效降低SAP三酰甘油水平并减低炎症反应、改善病情。Objective To observe the effects of early low fat enteral nutrition(EN) on serum triglyceride in patients of severe acute pancreatitis(SAP).Methods A total of 34 SAP patients accompanied by hyperlipemia were randomly divided into trial group(n = 16) and control group(n = 18),and early EN was supplied via jejunum nutrition tube confirmed by endoscopy.The trial group was administered with low fat EN while the control group with standard nutrient solution via EN.The changes in blood triglyceride,white cells,C reactive protein,aspartate aminotransferase,calcium and glucose were observed before and after administration.Results The levels of triglyceride,white cells,C reactive protein,aspartate aminotransferase and calcium decreased obviously in the two groups after EN administration(P < 0.05),but more obvious decrease was found in the trial group than in the control(P < 0.05).The level of blood glucose also decreased obviously in the two groups after EN administration(P < 0.05),between which there was no significant difference probably due to insulin treatment(P > 0.05).Conclusion Low fat EN administration can effectively decrease serum triglyceride,degrade inflammatory reaction and improve condition of SAP patients.
