TC模式下打击乐对女性甲基苯丙胺依赖者情绪障碍的干预治疗

摘　　要：目的:通过节奏动力强的打击乐治疗来观察治疗者的体验和表达的强烈情绪,进而降低甲基苯丙胺依赖者的焦虑抑郁程度;通过形成生动、层次丰富的音乐表现力,提高治疗者配合个人和团体不同的情绪特征和动力使用能力,观察甲基苯丙胺依赖者的人际交往能力以及冲动控制能力的提高程度等。通过治疗使病人宣泄情感,稳定情绪,学会控制和管理自我情绪,改善临床症状,保证监所安全,对恢复人际功能、降低性犯罪危险系数、促进社会和谐稳定将有深远的现实意义。方法:从2012年1月至2012年6月入北京市公安局强制隔离戒毒所向日葵治疗社区的93名女性甲基苯丙胺依赖者伴发情绪障碍人群中,采用随机抽样原则抽取60名,随机分成两组:试验组和对照组,各30名。试验组采用打击乐治疗2个月,对照组不进行打击乐治疗,应用症状自评量表(SCL-90)、汉密顿焦虑量表(HAMA)以及汉密顿抑郁量表(HRSD),动态观察打击乐对甲基苯丙胺依赖者情绪障碍的治疗效果。结果:试验组能明显降低女性甲基苯丙胺依赖者SCL-90中的强迫、抑郁和焦虑因子分(P<0.001),降低HRSD总分和焦虑、认识障碍、迟缓、绝望感(P<0.001)和睡眠障碍因子分(P<0.05),体重和日夜变化无显著差异(P>0.05),降低HAMA总分和精神性焦虑(P<0.001)以及躯体性焦虑因子分(P<0.01)。而对照组焦虑抑郁情绪则无明显变化,两组表现出明显的差别。结论:打击乐治疗能明显改善女性甲基苯丙胺依赖者的强迫焦虑抑郁症状,安全性高,依从性好,值得临床推广使用。Objective：To observe the experience and expression of strong mood in female methyl amphetamine（MA） addicts treated with strong rhythm and powered hitting music,and in this way to reduce the anxious and depressed degree in these addicts.To raise the utilizing ability of the treated addicts in different emotion characteristics during coordinating with individuals and teams by lively and multi-level musical expression.A improvement in interpersonal skills and impulsive control was observed after the musical treatment.This treatment could make the treated persons express emotions,stabilize mood,learn to control and manage their emotions,improve the clinic symptoms,and ensure a supervised place safety.It is reliable and far-reaching significant in resuming interpersonal functions,reducing sex crime rate,and promoting social harmony and stability.Methods： Sixty female MA addicts from Beijing Public Compulsory Isolated Detoxification Sunflower Treatment Community were randomly picked from 93 patients and were divided into two groups： the experimental group and control group.They were in Compulsory Isolated Detoxification from January to June in 2012.The experimental group was subjected to hitting music treatment,while the control group was not.All patients were scored with the Symptom Checklist-90（SCL-90）,Hamilton Anxiety Scale（HAMA） and Hamilton Rating Scale for Depression（HAMD） for two months.Results： Compared to the control group,the total scores of protracted withdrawal syndrome,HAMA and HAMD of the experimental group were significantly decreased after two month treatment.The compulsive,depressed and anxious scores of SCL-90,the scores of anxiety,cognition disorder,slow,despair feeling and sleep disorder of HRSD,the factor scores of body anxiety and spirit anxiety of HAMA of experimental group also showed significant decrease after two months.Conclusion： Hitting music treatment can alleviate the compulsive,anxious and depressed syndromes in female MA addicts.This therapy is efficient and sa
