经后入路双钢板内固定治疗肱骨髁上髁间骨折

摘　　要：目的:探讨经后入路双钢板内固定治疗肱骨髁上髁间骨折的临床疗效。方法:2008年7月～2012年6月间收治的33例肱骨髁上髁间骨折采用经后正中入路纵行劈开肱三头肌及其止点显露骨折,并行骨折复位內外侧双钢板坚强内固定术,术后早期功能锻炼。结果:33例患者术后获得6～26个月(平均18.4个月)月随访。全部患者均在术后6个月內愈合。肘关节功能评估按Cassebaum评分:优11例,良17例,一般5例,优良率为84.8%。术后创伤性关节炎2例,无尺神经损伤、异位骨化、伤口感染、内固定断裂等并发症。结论:经后入路双钢板內固定治疗肱骨髁上髁间骨折,固定牢固,操作方便,创伤小,术后可早期活动关节,肘关节功能恢复良好,疗效满意。Objective：To explore the surgical effect of posterior double plate fixation on humeral supracondylar and intercondylar fracture. Methods：33 cases of humeral supracondylar intercondylar fracture were operated by longitudinal splitting of triceps and its insertion by the median approach,then follwed fracture reduction,and internal and external side double plate fixation,early functional exercise were also encouraged operation.Results：After 6 -26 months（average 18.4 months） follow-up,All patients was healed and the time of fracture union was 6 months.According to the scoring system of Cassebaum：11 cases were excellent,17 case good and 5 case poor,the rate of excellent and good is 84.8%,2 cases had postoperative traumatic arthritis.No ulnar nerve injury,heterotopic ossification,wound infection,fracture of internal fixation.Conclusion：Posterior double plate fixation might be effective method for humeral supracondylar and intercondylar fracture,the fixation is firm,ease to operate,less trauma,contribute to early active joint,and have a better recovery of elbow functional.
