摘　　要：目的:探讨肱骨近端锁定接骨板治疗肱骨近端骨折的临床疗效。方法:回顾分析2007年4月～2012年4月使用肱骨近端锁定接骨板治疗肱骨近端骨折38例,其中男16例,女22例,年龄47～81岁(平均62岁),按Neer分类法:2部分骨折21例,3部分骨折17例。手术采用三角肌、胸大肌间隙入路,术后第3天肩关节适量被动活动,1周后适量主动及被动活动。结果:随访时间3～21个月,平均12个月,均达到骨性愈合,平均愈合时间为19周。按照Neer评分标准,功能优24例,良9例,中5例,优良率为86.8%。有1例患者LPHP固定失败。结论:对于肱骨近端骨折的患者,该手术切口创伤小,血运破坏少,肱骨近端锁定接骨板可起到牢固固定肱骨近端骨折的作用,特别适用于肱骨近端骨质疏松性骨折。Objective：To summarize the clinical effect of open reduction and internal fixation with proximal humeral internal locking system to treat the proximal humeral fractures.Methods：Retrospective study of 38 cases of proximal humeral fractures treated with locking plates from April 2007 to April 2012.There were 16 males and 22 females,with age of 47 -81 years old（mean 64）.According to Neer classification,21 cases with two -part fractures,17cases with three part fractures.Reduction and fixation was done via deltopectoral -groove approach,Shoulder exercise be performed passively three days after operation,passive and active shoulder exercise was done after one week operation.Results：The follow up period was 3-21 months（mean 12 months） on 22 cases showed that all fractures were healed and union period of 19 weeks.According to the Neer shoulder score,the results showed 24 excellent,9 good and 5 fair,with excellence rate of 86.8%.Conclusion：Humeral internal locking system can firmly fixate the fractures,as simultaneously reaches fracture healing and function exercise and enhances the treatment effect.The locking plate might be specially suitable for osteoporosis and comminuted proximal humeral fractures.
