卵母细胞体外成熟在卵巢低反应患者中的临床应用

摘　　要：目的探讨体外受精(in vitro fertilization,IVF)周期控制性卵巢刺激(controlled oocyte stimulation,COS)过程中的卵巢低反应(poor ovarian response,POR)患者,及时补救性应用未成熟卵母细胞体外培养技术(in vitro maturation,IVM)的有效性及安全性。方法对16例卵巢低反应患者补救性应用IVM技术。结果共取卵子225枚,其中未成熟卵子196枚,体外培养24 h后成熟100枚,受精69枚,卵裂65枚,移植胚胎27枚。同时获取成熟卵子36枚,受精29枚,卵裂26枚,移植胚胎8枚。移植周期14例,放弃周期2例;妊娠7例,其中宫外孕1例,流产1例,分娩5例,出生胎儿7名(2男、5女),均健康。结论对于POR患者补救性应用IVM技术可以获得较好的临床结局。[ Objective ] To evaluate the efficacy and safety in women with poor ovarian response treated by res- cued in vitro maturation. [Methods] Clinical outcome of 16 patients were rescued with IVM technology. [Results] The total eggs were 225, among them 196 were immature oocytes. After 24 hours culture, lO0 eggs matured, fertilized egg number 69, cleavaged number 65, the number of transferred embryos were 27. While the acquisition of mature eggs were 36, fertilized 29, cleavage 26, transfered embryos 8. Transfer cycles in 14 cases, 2 cases gave up cycle; pregnancy in 7 cases, including 1 cases of ectopic pregnancy, 1 cases of abortion,5 cases of delivery, birth in 7 fe- tuses （2 male, 5 female） were healthy. [ Conclusion] For patients with poor ovarian response, application of rescued IVM technology can obtain well clinical pregnancy rate.
