摘  要:目的通过检测2型糖尿病(Type 2 diabetic,T2DM)患者血清中血管生成素样蛋白3(Angioprotein-like protein 3,ANGPTL3)的浓度并与对照组进行比较,探讨该蛋白与T2DM的关系,及其在T2DM患者中的影响因素。方法随机选取青海大学附属医院2012年6月至11月诊断为T2DM的住院患者55例为病例组,同期在体检中心体检的25名非糖尿病者为对照组,采用ELISA法测定血清ANGPTL3的浓度。结果 1.病例组空腹血糖(FPG)、尿素氮(BUN)及血清ANGPTL3水平较对照组高,差异具有统计学意义(P<0.001);高密度脂蛋白(HDL)较对照组低,差异具有统计学意义(P<0.001)。2.相关性分析显示:血清ANGPTL3与HDL、白蛋白(ALB)呈负相关(r分别为-0.384,-0.220;P值分别<0.01、0.05);与FPG、BUN及尿糖呈正相关(r分别为0.373、0.452、0.317、0.381;P值分别<0.05、0.01、0.01、0.01)。3.多元逐步回归分析显示:FPG、HDL、BUN是影响血清ANGPTL3的主要因素;T2DM患者中FPG、血尿酸(BUA)、非酒精性脂肪肝(NAFLD)是血清ANGPTL3主要影响因素。结论 ANGPTL3作为糖代谢与脂代谢的连接点,可能与T2DM糖脂代谢、BUA、NAPLD关系密切。进一步了解其作用及调节机制可能对T2DM的预防和治疗具有重要意义。Objective To investigate the serum level of angiopoietin - like protein 3 ( ANGPTL3 ) in patients with type 2 diabetes mellitus (T2DM)and the influencing factors for it. Methods Fifty five patients with T2DM were selected from the Qinghai university affiliated hospital in June 2012 to November 2012 as the case group,25 non- diabetic volunteers in the healthy center at the same period as the control group. ELISA was used to determine the concentration of serum ANGPTL3 and then compared and analyzed. Results 1. The fasting blood glucose ( FBG), blood urea nitrogen(BUN) and serum ANGPTL3 level of T2DM were higher than those of control group; high -den- sity lipoprotein( HDL), albumin ( ALB, platelet count ( PLC ), White blood cell count ( WBC ) and aspartate amin-otransferase( AST)were lower than those of control group; the differences had statistical significance(P 〈 0.001 ), 2. Correlation analysis showed that the serum level of ANGPTI_3 was negatively correlated with HDL and albumin ( ALB ) ( r: - 0.384 ,P 〈 0.01 ; r: - 0. 220 ; P 〈 0.05 ) ; and was positively correlated with FPG, urine glucose and BUN ( r:0. 452, P 〈 0.01 ; r: 0.317, P 〈 0.01 ; r: 0.381, P 〈 0.01 ). 3. Multivariate stepwise regression analysis showed that FPG, HDL and BUN were the main affecting factors for the serum ANGPTI3 ; FPG, non - alcoholic fatty liver disease (NAFLD) and blood serum uric acid (BUA) were the main affecting factors for the level of serum ANGPTL3 in patients with T2DM. Conclusion The level of serum ANGPTL3 is significantly higher in patients with type 2 diabetes mellitus than the control group and it shows a close relationship with metabolism changes of glucose and lipid. It indicated that ANGPTL3 may play an important role in the pathological course of T2DM.
