中青年人群血尿酸水平与臂-踝脉搏波传导速度关系的研究

摘　　要：目的探讨中青年人群中血尿酸(SUA)水平与臂-踝脉搏波传导速度(baPWV)的关系。方法对879例中青年健康体检人员的断面资料进行分析。依SUA水平四分位分组方法将男性627例、女性216例健康体检人员均分为4组。以直线回归法分析SUA水平与各临床指标的关系,以多元Logistic回归分析观察baPWV≥1 400 cm/s与临床指标的关系。结果无论男性和女性,baPWV及颈动脉内膜-中膜厚度(IMT)随着SUA水平升高而增加。尿酸与baPWV、IMT呈显著正相关(男性r值分别为0.201、0.118,P值均<0.01;女性r值分别为0.191、0.182,P值均小于0.01)。baPWV≥1 400 cm/s与SUA独立相关(男性相对危险度=1.003,P=0.006;女性相对危险度=1.004,P=0.004)。结论高尿酸血症可导致动脉弹性变化,中青年人群中SUA水平是早期动脉粥样硬化的独立危险因素。【Objective】 To investigate the correlation of serum uric acid level and brachial-ankle pulse wave velocity（baPWV） in young and middle-aged population.【Methods】 Cross-sectional data from 879 individuals（627 case in male,216 case in female） in young and middle-aged population who underwent general health screening were analyzed.Sex-specific quartiles of serum uric acid were used.Correlation between serum uric acid（SUA） level and clinic parameters were obtained using linear regression analysis,and the logistic regression analysis was performed to identify independent predictors of baPWV ≥1 400 cm/s.【Results】 BaPWV and carotid intima-media thickness（IMT） showed a great increase according to the SUA quartiles in both genders.The person＇s correlation coefficient for the relationship between SUA levels and baPWVs and IMT were significant in both genders（r =0.201 and 0.118,P 0.01,in males;r =0.191 and 0.182,P 0.01,in females,respectively）.Logistic regression analysis showed that SUA level was an independent risk factor for baPWV≥1 400 cm/s（RR =1.003,P =0.006,in males;RR =1.004,P =0.004,in females）.【Conclusion】 Hyperuricemia can accelerate the development of arterial stiffness.SUA level is susceptible to early artery atherosclerosis in young and middle-aged population.
