注水减轻结肠镜检查患者腹痛的随机对照研究

摘　　要：目的明确注水结肠镜对结肠镜检查患者腹痛程度的影响。方法非麻醉结肠镜检查的门诊患者400例,随机分为左半结肠灌注温水法(注水组)和传统注气法(注气组),比较两组患者腹痛评分、进镜时间等差异。结果注水组10min内到达回盲部成功率为78%,明显低于注气组的87%。注水组与注气组进镜时间([8.01±1.26)vs(7.66±1.58)min],退镜时间([5.93±2.30)vs(4.80±2.03)min]及操作总时间([13.94±2.74)vs(12.45±2.58)min],差异均有显著性。注水组腹痛评分(2.23±1.71)明显低于注气组腹痛评分(2.87±1.94)(P=0.002)。两组之间息肉或腺瘤发现率、下次肠镜是否要求麻醉,差异无显著性。结论注水结肠镜是一项降低结肠镜检查患者腹痛的简单、有效方法。【Objective】To confirm the effect of warm-water infusion in lieu of air insufflation during unsedated colonoscopy examination on the incidence of abdominal pain and technical difficulties. 【Methods】400 outpatients for unsedated colonoscopy were randomly assigned to the group of warm-water infusion in left-sided colonoscopy （water group） and traditional air insufflation group （gas group）. The abdominal pain score and cecal intubation time were compared between water group and gas group. 【Results】The cecal intubation rate within 10 min of water group was 78% , which was significantly lower than 87% of the gas group. Between the water group and the gas group, the time to cecal [（8.01±1.26） min vs. （7.66±1.58） min], the withdraw time [（5.93±2.30） min vs. （4.80±2.03） min] and total procedure time [（13.94±2.74） min vs. （12.45±2.58） min] were of significant difference. The abdominal pain score of water group （2.87±1.94） was significantly lower than the gas group （2.23±1.71） （P =0.002）. Polyp or adenoma detection rate, and the rate of asking for sedation assumed next colonoscopy, were not significant different between the two groups. 【Conclusions】Warm water infusion for unsedated colonoscopy is a simple and effective method to reduce the abdominal pain during colonoscopy.
