经提/切眉切口祛除眶脂及部分轮匝肌下脂肪垫在肿泡眼整复术中的应用

摘　　要：目的探讨经提/切眉切口祛除眶脂及部分轮匝肌下脂肪垫在肿泡眼整复术中的应用。方法 32例患者采用经提/切眉切口,祛除眶脂及部分轮匝肌下脂肪垫,合并泪腺脱垂者同期还纳复位,术后5～7 d拆线,随访3～18个月,观察术毕停工期、术后肿胀情况、切口瘢痕、手术效果。结果术后上睑肿胀较轻微,停工期1～5 d,平均2.3 d。术后5～7 d拆除缝线时水肿基本消退。术后随访3～18个月,入组患者上睑松弛均得到明显改善,17例鱼尾纹同时减轻。所有患者切口位于眉下或重新设计之文眉范围内,瘢痕几乎不可见,上睑中外1/3处膨隆消失,眼周显著年轻化。结论经提切眉/切口祛除眶脂及部分轮匝肌下脂肪垫,并矫正脱垂泪腺组织,从解剖学基础上彻底矫正肿泡眼畸形,损伤轻,恢复快,效果明显,值得临床推广。Objective To determine the effects of infraeyebrow blepharoplasty for bubble-eye deformi- ty by removing both preseptal fat tissue and fat pad under orbicularis oculi. Methods A total of 32 cases who receiving above mentioned surgical techniques in our department from December 2009 to August 2012 were enrolled and retrospectively analyzed in this study. The ectopic lacrimal gland was restored at the same time if it was discovered. In 5 to 7 d after surgery, the sutures were removed, and the patients were followed up for 3 to 18 monthes. Down-time, swelling, surgical scar and clinical outcomes were observed and recorded. Results In all patients, swelling was slight, and then disappeared in 5 to 6 d after removing the sutures. The down-time caused by surgery was only 2.3 d in average, ranging from 1 to 5 d. During the following-up, blepharochalasis was significantly corrected, and along with the mitigation of Crow＇s feet in 17 cases. All surgical scars were almost undetectable, as well as the baggy appearance with the loss of the distention in 1/3 out of upper eyelid. Conclusion infraeyebrow blepharoplasty for bubble-eye deformity by removing both preseptal fat tissue and fat pad under orbicularis oculi, along with restoring the ectopic lacrimal gland, is a effective surgical approach for correcting bubble eye deformity based on anatomy discovery, along with slight injury, rapid recovery and long- lasting effects.
