摘  要:目的研究纯音听阈正常的原发性良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)患者耳蜗功能状态。方法对纯音听阈正常的原发性BPPV患者25例(25耳,病例组)及正常人20例(40耳,对照组)行瞬态声诱发耳声发射(transiently evokedotoacoustic emission,TEOAE)及畸变产物耳声发射(distortion product otoacoustic emission,DPOAE)检测,记录TEOAE通过率、各频段重复率及DPOAE各频点检出率、幅值进行统计学分析。结果①病例组TEOAE通过率及各频段重复率较对照组减低,差异有统计学意义(P<0.05或P<0.01)。②病例组DPOAE在0.5、1.0、4.0 kHz频率的检出率以及所有频点幅值较对照组减低,差异有统计学意义(P<0.05或P<0.01)。结论部分原发性BPPV患者纯音听阈虽然正常,但其患耳耳蜗已存在早期病变,病因可能与内耳微循环障碍相关。对于这些患者应给予及时治疗,避免不可逆的耳蜗损伤。OBJECTIVE To investigate the cochlear function of affected ear in primary benign paroxysmal positional vertigo (BPPV) patients with normal pure tone threshold. METHODS 25 primary BPPV patients with normal hearing (25 ears) were set as case group while 20 normal volunteers (40 ears) served as controls. Transiently evoked otoacoustic emission (TEOAE) and distortion product otoacoustic emission (DPOAE) were detected respectively. The passing rate, each frequency repetition rate of TEOAE and detection rate, amplitudes at all frequencies of DPOAE were recorded and analyzed. RESULTS ~There were statistica~ differences in passing rate and all frequencies repetition rate between the two groups (P〈0.05orP〈0.01) .(~Compared with the control group , detection rate of DPOAE at 0.5 kHz, 1.0 kHz, 4.0 kHz and amplitudes at all frequencies in the case group were significantly decreased. There were statistical differences (P〈 0.05 or P〈 0.01) . CONCLUSION Although some primary BPPV patients has normal hearing but their cochlear function have been early damaged.The dysfunction of inner ear microcirculation is presumably related to it. These patients should be treated promptly to avoid irreversible damage in cochlea.
